Dear District 86 Community,

On Monday, April 5, we will be partnering with Osco to administer the single-shot Johnson and Johnson COVID-19 vaccine to individuals who fall into one or more of the categories currently being prioritized by the Illinois Department of Public Health. These categories include:
 
  • Health care personnel
  • Long-term care residents
  • Individuals who are 65 and older
  • Expanded essential workers in the 1B category
  • People with a disability
  • People with a specific health-complicating condition or status that include obesity, diabetes, pulmonary diseases, smoking, heart conditions, chronic kidney disease, cancer, solid organ transplant, sickle cell disease or pregnancy.
  • Government employees
  • Higher education staff members
  • News media services employees
 
The vaccinations will take place from 9 a.m. to 5 p.m. at Hinsdale Central (5500 South Grant Street in Hinsdale), and will be provided free of charge.
 
After sharing this information with people aged 65 and older who pre-registered with us in February and March, we still have slots available. To help ensure that none of the doses go to waste, we are extending this opportunity to our community on a first-come, first-served basis. If you or someone in your family lives in Illinois, is at least 18 years old, and falls under one or more of the eligible categories listed above, you can schedule an appointment via Osco's online portal by visiting http://bit.ly/3m6SZNz. Below are the steps you must complete in the portal when signing up for an appointment.

  • Click the box at the bottom of the first page on the portal to confirm that you fall under one or more of the eligible categories listed above. You can see what the box says and looks like by visiting https://bit.ly/3cBP9Zv. Please note that you will need to bring documentation to your appointment that verifies your eligibility. This documentation may include proof of employment (e.g., badge, pay stub, uniform, etc.), a medical license (if applicable), or a medical or prescription insurance card.


  • Select the time for your appointment. Please select a time that will allow for the 45 minutes needed to complete the entire process - i.e., arrive on-site, wait in line, present your documentation, receive the vaccination and be monitored for at least 15 minutes following the injection.

We kindly ask that you do not forward or share the link to the portal with others. We want to make sure that we have enough doses of the vaccine for eligible members of our community.

Below is information about the tasks you must complete before getting vaccinated, and the steps you will need to follow on the day of your appointment.

Steps to Complete Prior to the Vaccination 
Please complete the following tasks prior to your appointment. 

  • Print out and complete the Informed Consent for Immunization form. You will need to present this form to the staff from Osco the day of your appointment. If you are unable to complete the consent form in advance, there will be print copies available on-site when you come for your appointment.

  • If possible, please make a copy of the front and back of your insurance card. If you are unable to make a copy, please make sure you bring your card with you the day of your appointment.

Process and Logistics for Receiving the Vaccination
Below are the steps that everyone must follow on the day of their appointment. 

  • You will not need to take any precautionary steps or measures (e.g., fasting) prior to receiving your vaccination. However, you are asked to please wear clothing (e.g., a shirt with sleeves that are short or loose fitting) that will enable the staff member from Osco to quickly and easily administer the vaccine to the upper portion of your arm near the shoulder. 

  • In order to ensure that we can maintain a distance of at least six feet between those who are being vaccinated, we ask that you please come at your scheduled time. You will not be permitted to get in line or enter the auditorium if you arrive early for your appointment. 

  • If you are driving to Hinsdale Central for your appointment, please park in either the student or staff lots located on the southeast side of the school on Grant Street, and enter the building through Door 5.

  • When you arrive for your appointment, you must wear a mask and practice social distancing at all times. You must also have the following items with you:

  1. Informed Consent for Immunization form if you are able to complete it in advance
  2. Your insurance card - either a copy of the front and back or the actual card
  3. Photo ID (e.g., driver’s license, state ID, etc.)

  • You will check in with a member of the district’s security staff when you enter Door 5. You will then be directed to the auditorium where you will check in with the staff from Osco and present all of your documentation. After checking in with the staff from Osco, you will be asked to sit until your name is called. 

  • When your name is called, you will proceed to one of the vaccination stations and receive your injection. After receiving your injection, you will need to wait in the auditorium for approximately 15 minutes for monitoring purposes. When the 15 minutes is up, you will go through the doors at the back of the auditorium and exit the building via Door 10.

  • We will have staff members from GSF-USA on-site throughout the day to clean spaces such as the vaccination stations on a regular basis.
