Dear College Park Academy 6th-12th Graders, Family and Staff,
Continuity of Dental Care is vital to maintaining teeth for a lifetime and decreasing inflammation in the whole body. We are excited to partner with College Park Academy (CPA) to provide an alternative to the Mobile Dentist. A few benefits of providing Door to Door Dental Care to your teen are, not missing school for general Dental care, free shuttle from CPA to the Dental office, a safe waiting space for Teen pick-up and a after school lunch.
If you are interested in your teen receiving Dental Care or would like care yourself, please call our office at 301-345-5656 or text 301-318-5285 the following information:
Name, address, email and cellphone numbers of Parent and potential Teen patient
Best time to call you during the day and after 6pm
Child and Parent Date of Birth
Name of Insurance
Insurance phone number
Group number
Employee Identification number
If using insurance as your form of payment, once your insurance has been verified, the parent will be contacted by one of our team members to schedule an appointment for your teen. A notification for upcoming treatment will be sent to CPA a week prior, confirming the date the shuttle will be picking the teens up after school. If not using insurance, book directly HERE for an online appointment on a non-shuttle day or pre-pay for a shuttle day. We are open Presidents Day.
