Age-Friendly Care Practice Transformation
Applications Closing Soon! CTC-RI is excited to offer a funded quality improvement initiative focused on implementing the 4Ms Framework (What Matters, Medication, Mentation, and Mobility) and supporting care partners of people living with dementia.
Participating primary care practices will receive up to $6,000, one-on-one practice facilitation, and an opportunity to earn national Age-Friendly Health Systems recognition.
Applications are due July 31, 2026. Learn more and apply today.

Advance Your Care Management Skills with the GLearn Core Curriculum
Applications are now open for CTC-RI's funded Nurse Care Manager/Care Coordinator (GLearn) Core Curriculum Training Program.
Up to 10 Rhode Island primary care nurse care managers and care coordinators will receive access to a nationally recognized training program, weekly faculty-led learning sessions, and the opportunity to earn up to 39 RN CEUs and 36.75 CCM credits.
Applications are due August 14, 2026.

Advancing Breastfeeding Support Across Rhode Island ECHO® Series
Registration is now open for CTC-RI's Advancing Breastfeeding Support Across Rhode Island ECHO® Series.
This free virtual learning community brings together healthcare professionals to strengthen equity-centered lactation support, complex case management, and cross-sector collaboration.
Participants can attend individual sessions or enroll in the full series to receive a $250 stipend (eligibility requirements apply).
Register by August 14, 2026.

CTC-RI Launches Adult Immunization Quality Improvement Program
CTC-RI has launched a 10-month Adult Immunization Quality Improvement Project with seven Rhode Island primary care practices, funded by UnitedHealthcare. Using data-driven quality improvement, practice facilitation, motivational interviewing, and peer learning, the initiative will help practices improve influenza vaccination rates among adults ages 19–64 and strengthen preventive care ahead of the 2026–2027 flu season.

Quarterly PCMH Kids Stakeholder Meeting
30 participants joined our July 2nd quarterly meeting, Improving Child Health in RI: Learn How RI Certified Community Behavioral Health Clinics (CCBHCs) and Mobile Crisis Units Support Pediatric Care, which featured a distinguished panel including: Amy Hulberg, Ashley Reyes, EOHHS, Samantha Ryder, BHDDH, Christopher Strnad, DCYF, and Sarah Kelly-Palmer, FSRI.
