Yale Survey on Reporting Errors in Surgical Pathology

Yale University is conducting a survey on surgical pathology QA to better define diagnostic errors. We are told the brief survey takes about 10 minutes to complete and the survey responses should be submitted in the next two weeks. Here is a link to the survey;
