This year we are doing nominations in two phases. Please review so that you know which documents, details, and letters of recommendation are necessary to complete the online nomination form in Phase 1 & 2. Gather all these documents at your earliest convenience after completing the first phase.

deserves an award
Full Nomination Form (Phase 2) - Deadline 6-3-21
After membership eligibility has been verified you will complete a second form that asks for a Letter of Nomination, Letters of Recommendation, CV/Resume, Educational Philosophy (educator award), evidence of Research (researcher award).
