The end of the year at Caravan means an opportunity to meet with many of our clients, partners, and all of our employees at the Accountable Care Symposium. This year’s event in Phoenix was our largest yet, with 633 attendees including 120 Caravan employees and 513 doctors, nurses, and health system leaders committed to improving the care their patients receive.

Second Annual Population Health Pioneer Awards
On Wednesday night, we presented the second annual Population Health Pioneer Awards. The organizational awards were based on quantifiable values. The individual awards spanned ACO, TCPI, and CPC+ and were based on more than 100 nominations that came from all of you.

2017 ACO-CAHPS Patient Satisfaction Surveys Are in the Mail!
Attention ACO Champions, Physician Leaders, CEOs, Practice Managers, Care Coordinators:
The ACO-CAHPS survey has been sent to CMS-selected ACO patients by our CMS-certified vendor, DSS Research! As a reminder, the survey focuses on the interpersonal aspects of patient health care experiences in the
outpatient medical office setting
(not the hospital) and counts for 25% of the ACO’s overall quality score.
Timeline:
- 12/12- 12/13: Second follow up survey sent to those who have not responded or have partially completed
- 1/4-2/1: Telephone follow-ups conducted with non-responders
- 2/6-2/8: Data is submitted to CMS

2018 ACO Provider/Supplier Lists – Missing Physician Assistant Data
Attention CEOs, Physician Leaders, ACO Champions, IT Contacts & Care Coordinators:
CMS has sent notification that the 2018 ACO Provider/Supplier List provided to you on December 1, 2017 does not include physician assistant employment arrangements. Please continue to review and modify all of your other ACO providers and suppliers included on your list and certify your 2018 ACO Provider/Supplier List by the
December 13, 2017 deadline to complete Annual Certification.
In January, they will create a supplemental Provider/Supplier List to include the physician assistant data and deliver the revised file to your ACO. At that time, you will need to review your Provider/Supplier data and again re-certify your 2018 ACO Provider/Supplier List. Please contact your Account Manager for questions.
