Reminders
Use Your Onboarding Checklist
Continue to reference your
onboarding checklist, posted on your
transition website, to stay on top of your onboarding activities. The transition website is a temporary resource and will be deactivated at the end of September. Be sure to download any resources you'd like to keep before then.
Prioritize Completing Form I-9
For employees transitioning to CareMount on Sept. 1, the Form I-9 was completed electronically through UKG onboarding. The final step is to provide your manager with a copy of your official documents; it is your manager’s responsibility to send the documents to the CareMount HR team.
Complete this as soon as possible — by law, both sections of your Form I-9 must be successfully submitted by 4 p.m. ET on Sept. 6 otherwise, you will not be able to continue employment with CareMount.
Review FAQs
We've added more questions and answers to the master FAQ document on the transition website, including a new item on 401k. We recommend that you continue to review the FAQs on your
transition website.
Additional Resources
We’ve added a list of
CareMount resources for HR and payroll questions that is available now on the
transition website.
After Sept. 1, additional resources and links directing you to UKG will be available on the ProHEALTH NY intranet.
As a reminder, if you'd like to review any of the previously distributed messages, please reference the Message Archive on your
transition website (enter CareMount2022 as the case-sensitive password).
If you plan to be out of the office at the time you have an onboarding activity (e.g., PTO or a leave of absence), please contact
Meaghan Slater immediately.
Thank you,
