Go Live Announcement for the Care and Case Management (CCM) system
The Department of Health Care Policy & Financing (the Department) is pleased to announce the new CCM system will Go Live in April 2022.

Prior to Go Live, we will work with the case management agencies (CMAs) participating in the Soft Launch to test the functionality of the new Assessment and Person-Centered Support Plan in the new CCM system. Participating case managers will work with members who volunteer to go through the new Assessment and Person-Centered Support Plan during their annual Continued Stay Review (CSR). The Soft Launch will provide valuable data and feedback to the Department before the new system and Assessment and Person-Centered Support Plan are implemented for all case managers and members.

The Department is working to develop training for all case managers which will be available before April 2022. System training will be available through Gainwell Technologies and comprehensive training on the new Assessment and Person-Centered Support Plan will be available through the Department.

Opportunity to Provide Input on Case Management Redesign
As part of the Case Management Redesign initiatives for HCBS waivers, the Department will be hosting two virtual listening sessions for HCBS case management staff to gather information and feedback on current and future definitions of quality case management. Please register for ONE of the following sessions:

Tuesday, Oct. 5, 2021

or

3:00 p.m.-5:00 p.m.
Wednesday, Oct. 6, 2021

During each meeting, Department staff will provide a brief presentation on the Department's efforts to redesign the case management system, then case managers will participate in small, randomized group discussions. The feedback received during these listening sessions will be collated into themes and used by the Department in their decision-making on key policy areas for Case Management Redesign.

Alternatively, if you are unable to attend one of the dates listed above, the Department has created a survey to gather information and feedback from all stakeholders, including case managers, on current and future definitions of quality case management.
