CCM Job Aids, Known Issues and Release Notes

Job Aids, Release Notes, and Known Issues can be found in the CCM Documentation link under your user profile in the CCM System. The Job Aids, Recordings, and Release Note documents can also be found in the Training Google Drive.


HCPF anticipates the removal of all proprietary information (including job aids, recordings, and release notes) from the Training Google Drive by August 31, 2024, so please begin using the CCM Documentation link to access these documents, including Job Aids.

Please follow the steps below: 

  1. Log in to the CCM system
  2. Click on your name
  3. Click on CCM Documentation
  4. A new browser tab opens up with the Colorado CCM Documentation Landing Page document
  5. Click on the "pages" blue arrow to select Page 4 for the Known Issues document
  6. Click on the "pages" blue arrow to select Page 5 for the Training Materials document:
  7. Click on the Job Aids link OR 
  8. Click on the User Friendly Release Notes link.

Other News for Case Managers

Presumptive Eligibility for Long Term Supports and Services 

Presumptive Eligibility for Long-Term Supports and Services (LTSS PE) is scheduled to be implemented on January 1, 2026.


Here are some details about this new program:

  • LTSS PE will support members who are experiencing defined crisis circumstances
  • Individuals will self-attest to eligibility through an online referral form, and have access to a few select LTSS services right away
  • Long Term Care Medicaid eligibility will be processed following the current timelines
  • During the LTSS PE period, a referral will be sent to Case Management Agencies so the full Level of Care assessment, and other intake steps, can be completed following current timelines
  • Colorado is implementing this program through the expansion of our 1115 demonstration waiver

New Family Support Services Program (FSSP) Resource available to Case Management Agencies (CMAs)

HCPF has developed a FSSP Guidance Document to assist CMAs with completing the FSSP Program Evaluation and FSSP Annual Report which are both due to the Department each year. The FSSP Guidance Document can be found on the FSSP LTSS website.

For User Access to CCM, Bridge, PEAKPro, please complete a 3rd Party System User Access Request Form, to include obtaining required signatures from your agency management, and submit to HCPF_OCLSystemApplications@state.co.us.
