Streamline Eligibility and PEAKPro Training

Streamline Eligibility Implementation and PEAKPro training occurred this month. A recording of the Streamline Eligibility Implementation training and a copy of the slide deck will be posted to the LTSS training webpage shortly after the last session of this training concludes. This will also occur for the PEAKPro training once the sessions are completed. 


Job Aids have been updated based on the Streamline Eligibility Implementation in the CCM system. These job aids are available in the CCM System Job Aids and Resources Google Drive. Our CCM users have received a full list of which materials have been updated and which ones are new through the email blast going directly to users.

To request support for a CCM system issue, case managers should call the CCM Support Center (888) 235-6944 or complete and send a CCM Support Request Form attached to an encrypted email to ccmsupport@assurecare.com.


If you need to request user access to CCM and Bridge, please complete a 3rd Party System User Access Request Form, including the required signatures from your agency management. Submit the form to HCPF_OCLSystemApplications@state.co.us

CMA/DVR Roundtables

All case managers, supervisors, and staff responsible for referrals are invited to attend the upcoming Roundtable sessions that are hosted by the Department of Health Care Policy and Financing (HCPF) and the Division of Vocational Rehabilitation (DVR). The objective of the Roundtable sessions this year is two-fold: 1) Education and 2) Networking.


There are two remaining sessions that align with the CMA Transition Phases. Each session will run from 10 a.m. to noon.



 What to Expect?


  • Brief update related to Case Management Redesign
  • Training on the intersection between DVR and DD/SLS Waiver Supported Employment
  • Networking opportunity with the DVR office(s) that serves your designated area
  • Confirm/establish contacts between your organization and DVR office(s)
  • Share your intake process
  • Outline your current (if any) referral process to DVR


Representation from CMAs is critical to the success of the Roundtable sessions. Thank you in advance for your participation and partnership! Ensuring CMAs have appropriate contacts with DVR and staff understand the sequencing of funding that can exist between DVR and CMAs will be invaluable in supporting people with intellectual and developmental disabilities in their employment goals.

Public Health Emergency (PHE) Unwind

PHE Unwind Reporting

HCPF publishes renewal unwind data reports and summaries on the Continuous Coverage Unwind Data Reporting page. Information in the reports is point-in-time data about renewals as required by federal reporting requirements and does not include new members coming into the program or those who will take action during the 90-day reconsideration period. Members can return their renewal packet and any missing information to their county for processing within 90 days of losing coverage and ask for retroactive coverage if they experience a gap. 


PHE Unwind Support 

Please help us spread the word to Health First Colorado and CHP+ members to get ready for renewals by sharing the messaging our partner toolkits: Update Your Address and Take Action on Your Renewal. A new joint webpage, KeepCOCovered.com, also includes partner resources and information for those who may need to transition to other coverage. 


Member-facing materials are available in English and Spanish and have been transcreated into the top 11 languages spoken by members: Amharic, Arabic, Burmese, Chinese, Dari, Pashto, Russian, Somali, Swahili, Ukrainian, and Vietnamese.

HCPF hosts quarterly webinars on the PHE Unwinding. These webinars are geared toward community partners such as advocacy organizations, providers, and community organizations who may provide other assistance to Health First Colorado or CHP+ members (housing, social services, etc.). ARPA 5.04 CMA/RAE Care and Case Management Best Practices Project Update

HCPF, with the support of the contractor John Snow Research and Training Institute, Inc. (JSI), is conducting a project to improve communication, collaboration and coordination for members receiving long-term services and supports (LTSS) who are assigned to a RAE and a Case Management Agency (CMA).


JSI’s findings for best practices can be categorized in five domains all of which contribute to the overall goal of improving member experience. The following table lists the best practices by domain/category:

Learning Collaborative Series for Colorado HCBS Case Managers

As we navigate change and opportunity in the Colorado Home and Community-Based Services (HCBS) system, including Case Management Redesign, this monthly series of 50-minute discussions aim to provide individual Colorado HCBS case managers the opportunity to consider new strategies, gain knowledge, and enhance strengths – especially during times of transition. Each session will offer a live presentation, attendee activities, group discussions, and time for questions and answers regarding the monthly topic. Please email any questions or concerns.

Stakeholder Kickoff Meeting: Expanding Health Related Social Needs Services

HCPF is beginning our work to engage stakeholders about how Health First Colorado (Colorado's Medicaid program) and Child Health Plan Plus (CHP+) can better support the health-related social needs (HRSN) of our members. A kick-off meeting will be hosted to present on HRSN initiatives underway at HCPF, national opportunities and collaborations across the state, and to collect input from attendees on their priorities.


All stakeholders are welcome, including those who perform social needs assessments and referrals, or those who provide or receive those services.

The Wellness Education Benefit Case Manager Trainings

The Wellness Education Benefit (WEB) will be educational materials mailed monthly to Children’s Home and Community Based Service (CHCBS) waiver members. The WEB is defined as individualized educational materials designed to prevent hospitalization or movement into an institutional setting. The WEB helps members obtain, process and understand information that assists with managing health-related issues, promoting community living, and achieving goals identified in their person-centered service plans. The WEB also provides members with actionable tools and information for informed decision-making and preparation for conversations with medical and social service professionals.


This benefit will be available to members in May 2024. Information surrounding the WEB will be important for case managers so they can assist in discussing the benefits of the WEB with waiver members. The WEB is an important service for members and may support continued waiver eligibility.
