Monthly update meetings continue to be held with CMA leadership to provide updates and information, collaborate on implementation planning, and respond to questions. These meetings occur monthly at the quarterly directors' meeting and on the last Tuesday of the month in months where there is no quarterly meeting scheduled. You can reach out to your agency leadership for these updates as well as continue to look for them here. 

Soft Launch Participant Office Hours
For this format, we will request questions/topics in advance and provide any planned agenda topics in advance. Individual attendance can be determined based on the topics being covered.

If we have information or topics to share that we believe will be beneficial to the whole group, we will use these planned times to provide that information and will provide advance notice of the changed format and content.

Myths and Facts about Change to Case Management

Attendees will learn about Colorado's Case Management Redesign and the factors that led to the proposed changes. This session is for case managers and will offer a live presentation by leadership from the Department of Health Care Policy & Financing and a moderator-led interview. Attendees will participate in small group breakout activities and a group discussion along with question and answer times.

Home Modification Benefit Checklist

The Home Modification Team has developed a new Home Modification Checklist Tool for Case Managers to use. This checklist will help to guide the process and the necessary documentation for Home Modification projects. Please download this new tool and begin adding this to your process. We are also working on updating our current OT/PT Home Modification Evaluation Form. We will send an update in this newsletter once this form is posted as well. Please remember that the Home Modification Team is offering training for Community Centered Board (CCB) and Single Entry Point (SEP) Case Managers quarterly now. As a reminder, when a member has exhausted their Home Modification Benefit, they are eligible to access an additional $10,000 per member in American Rescue Plan Act (ARPA) funding until March 2024. In order for a member to access this funding, they will need to go through the Home Modification process that includes a packet submission to the Department of Housing (DOH).
