CCM Job Aids, Known Issues and Release Notes - NEW!

CCM end users can access the job aids, Known Issues document and Release Notes from the CCM system. Please utilize the instructions below to access the new CCM Documentation section to review the User Friendly Release Notes for Stabilization Release 8. These changes were implemented in the CCM on July 21, 2024. 


Please follow the steps below: 

  1. Log in to the CCM system
  2. Click on your name
  3. Click on CCM Documentation
  4. A new browser tab opens up with the Colorado CCM Documentation Landing Page document
  5. Click on the "pages" blue arrow to select Page 4 for the Known Issues document
  6. Click on the "pages" blue arrow to select Page 5 for the Training Materials document:
  7. Click on the Job Aids link OR 
  8. Click on the User Friendly Release Notes link.

Training for New CCM Users

Last month, AssureCare introduced a new training offering for Case Managers who are new to the CCM system. This program is designed to equip Case Managers with the necessary knowledge and skills for their roles with no prior CCM system experience. Here are the Learning Curriculum Instructions with the program details.

Case Managers and CCM Users must access CCM Job Aids and CCM System recordings to help them complete their processes, tasks, and documentation in the CCM System.

Telligen Office Hours

Telligen is hosting office hours for case managers to answer questions regarding Utilization Review Utilization Management (URUM) reviews for:

 

  • Health Maintenance Activities (HMA)
  • Over Cost Containment (OCC)
  • Supported Living Services (SLS) Exception
  • Children's Home and Community-Based Services (CHCBS)
  • Children's Extensive Services (CES)


Please submit any office hours topic requests by using the Telligen Support Request form no later than August 2, 2024. 

 

When: August 15, 2024, 11:30 a.m. to 12:30 p.m.

Register and join on Zoom.

 

Each attendee should register individually. Questions and answers from previous office hours are on our webpage under Telligen Office Hours.

Increase in Over Cost Containment Threshold

On September 1, 2024, the Over Cost Containment Threshold will change from $315 average daily cost to $330 average daily cost. The Case Manager approval limit will not change.

New SLS Exception Review Request Form

A new SLS Exception Review Request Form will be effective on September 1, 2024. All SLS Exception Requests must use this new form effective this date and filled out in its entirety before review submission.


The Form will be available on the SLS Exceptions Website.

LTSS Presumptive Eligibility

HCPF is developing a new presumptive eligibility program for the long-term services and supports population. This program will be available in January 2026.


  • Presumptive eligibility for the Long Term Services and Supports (LTSS) population will allow eligible individuals in an identified crisis to self-declare eligibility.
  • Targeted LTSS services will be available immediately while an individual’s Long Term Care Medicaid application is processed.
  • HCPF is asking the Center for Medicare and Medicaid Services (CMS) to expand our state’s 1115 demonstration to include LTSS PE.
  • This summer we’ll be gathering program feedback to help us shape program details.

For User Access to CCM, Bridge, PEAKPro, please complete a 3rd Party System User Access Request Form, to include obtaining required signatures from your agency management, and submit to HCPF_OCLSystemApplications@state.co.us.
