Soft Launch Update

The Department of Health Care Policy & Financing (the Department) has decided to postpone the soft launch of the Care and Case Management (CCM) system and new Assessment and Person-Centered Support Plan (A/SP). As a reminder, the soft launch was planned to only impact the case management agencies (CMAs) that originally participated in the A/SP pilot. The soft launch will still take place. We will be in contact with applicable CMAs with more details. 

As we are postponing, the Department has extended the opportunity for additional CMAs and their case managers to participate in the soft launch.  

If your CMA is interested in participating, please notify NaDean Smith of their interest. The email should include the name and email address of the primary contact for soft launch communications and up to three seasoned case managers who would like to participate. As of now, the following case management agencies will be participating:
Single Entry Point (SEP) Transition

On July 1, 2021, two Single Entry Point (SEP) agencies will transition to a new Case Management Agency. County Department of Human Service agencies should be prepared to receive and provide information regarding long-term services and support (LTSS) from and to all incoming agencies, as they had previously received from the outgoing agencies.
