Thank You for Your Partnership

Your continued engagement and conversation are important to us as we work to bring you a new Care and Case Management (CCM) system that better serves our members and makes your job easier on a daily basis. We understand change and uncertainty can be challenging and we appreciate your understanding as we embark on this journey together. 

We want to thank everyone who had the opportunity to attend one of our Kick-Off meetings last month. If you were not able to attend either of these meetings, you can view the recording or the presentation slides

We want to reiterate what was communicated at the Case Management Redesign (CMRD) bi-monthly executive directors meeting and the CCM Case Manager Kick-Off meetings: at this point, it is unlikely we will fully implement the Care and Case Management system on July 1, 2021, as planned. The Department of Health Care Policy & Financing (the Department) is not prepared to share a revised timeline at this time, but is working with all of our vendors to determine timelines for all of the interdependent process and systems changes required to achieve a successful implementation. We are committed to a thoughtful and collaborative implementation process, and will communicate a new timeline as soon as we have a definitive date. 

During both meetings, we received many questions, and if we were not able to address your questions during the meeting, we are working now to answer them and will post them on the FAQ section of our website

Please continue to ask questions and share your thoughts so we can do our best to minimize the impact on your workload throughout the transition. We expect that partnership with Case Management Agencies will bring us the best outcome with the least disruption. The project team is committed to providing opportunities for feedback and as a reminder, here is a list of those opportunities. Please feel free to share with colleagues who may not be aware of the following resources:

  • This newsletter and other occasional e-blasts through this mailing list. If you are not subscribed, please opt- in at the Department's website and enter your information then select “Case Manager’s Corner” to sign up.
  • Sign-up for e-blasts through Gainwell’s email lists by entering your information and selecting “11- case manager”.
  • Visit our new Care and Case Management System website. Some areas of this site are still under construction, and if there are any major updates to the site, we will let you know through this newsletter.
  • Office of Community Living (OCL) webinars
  • Regular case management communication meetings. Often, we will provide information to the leadership of case management agencies and rely on leaders to share information through appropriate channels.
  • Other regular provider communication channels, including the Provider Bulletin, Provider newsletters and the Provider Portal.

Request for Second Survey Response

Thank you to all the respondents to our Initial Case Manager Survey in November. We are using your feedback to construct our communications and advise the opportunities we provide for engagement. In the spirit of continuous collection of your feedback, we invite you to participate in a second survey. 

Access to the Care and Case Management System March Survey is available through the Department's website.

The survey will be open for two weeks and will close on Mar. 29, 2021. 
Help Us Spread the Word About This Course

Colorado is offering an exciting new free class for people with disabilities receiving home and community-based services (HCBS) to learn to lead their own Person Driven Plan. The class is designed for all people with disabilities and uses the principles of Universal Design for Learning (UDL) to make sure all participants can access the course.

This course is taught live via Zoom during the pandemic. Dates and more details are available on this flier. Come join us!
