Care and Case Management (CCM) System News

Streamline Eligibility Implementation Training Available

A recording of the Streamline Eligibility Implementation training and a copy of the slide deck are posted to the CCM System Training Folder. CCM system training is no longer available on the LMS.  


Case Managers and CCM Users must access CCM Job Aids and CCM System recordings to help them complete their processes, tasks, and documentation in the CCM System. These resources are listed in the CCM System Training Google folder. Please remember to access these documents directly from the folder, so that you are always accessing the most current information.

Children's Buy In Program Stakeholder Engagement Opportunity

The Health First Colorado Buy-In Program for Children with Disabilities allows families of children with a disability who qualify to 'buy into' Health First Colorado (Colorado's Medicaid Program) for that child. If your child and/or your family earn too much for Health First Colorado or Child Health Plan Plus (CHP+) your child may qualify. If your child does qualify, you may pay a monthly premium to be in the program. We base your monthly premium on your family's income.


Buy-In members, stakeholders, and partners are invited to attend this meeting to learn about upcoming eligibility policy changes to the Children's Buy-In Program.

ARPA 5.04 CMA/RAE Care and Case Management Best Practices Project Update

HCPF, with the support of the contractor John Snow Research and Training Institute, Inc. (JSI), is conducting a project to improve communication, collaboration and coordination for members receiving long-term services and supports (LTSS) who are assigned to a RAE and a Case Management Agency (CMA).


Last month we shared with you the best practices findings that fell into the following categories:


  • Intra-Inter-Agency Alignment and Collaboration
  • RAE and CMA Role Clarification, Collaboration and Coordination
  • Data Sharing and Technology
  • Payment and Performance
  • Contracts and Oversight
  • Member Experience and Satisfaction


After identifying best practices to improve coordination, communication and collaboration between the RAEs, CMAs and HCPF, JSI engaged in additional analysis and outlined recommendations on how to implement each of the best practices in the six categories listed above. The recommendations included outlining the roles and responsibilities and core competencies needed to implement and sustain the best practices. This phase of the analysis concluded with a map summarizing what the future system would look like if the best practices are implemented successfully. 

CMA/DVR Roundtables

The CMA/DVR Roundtable targeting Phase 3 case management agencies which was scheduled for May 21, will be postponed to later in the year. In light of the current status of the Case Management Redesign and acknowledging the magnitude of the challenges currently facing our members, the Roundtable will not be held until after all CMA Transitions have occurred. Please watch for a new Zoom registration link for Phase 3 CMAs and DVR staff. 


DVR and HCPF Supported Employment staff will still be available from 10-11 a.m. on May 21, 2024 to answer any questions related to supported employment. The original Zoom registration link may be used to access this Open House style session, all Case Managers, DVR Counselors, and Providers are welcome to attend. Telligen is hosting office hours for case managers to answer questions regarding Utilization Review Utilization Management (URUM) reviews for: 

 

  • Health Maintenance Activities (HMA)
  • Over Cost Containment (OCC)
  • Supported Living Services (SLS) Exception 
  • Children's Home and Community-Based Services (CHCBS) 
  • Children's Extensive Services (CES)

Acupuncture, Chiropractic, and Massage Therapy on the Complementary and Integrative Health (CIH) Waiver Single Service Caps Removed

Eligible members on the CIH Waiver can now receive up to 408 combined units of acupuncture, chiropractic care, and massage therapy without any caps on individual services or single treatment units. 


For eligible CIH Waiver members, case managers may prior authorize any combination of these three services up to the 408 combined unit cap after recommendation by an enrolled provider. Members can choose from multiple providers for these services during their person-centered support planning process.


The updated rules are at CCR Sections 8.7502, 8.7512, and 8.7530.

Children’s Extensive Support (CES) and Children’s Habilitation Residential Program (CHRP) Skilled and Therapeutic Respite - Educational Webinar

Effective March 1, 2024, the CES and CHRP waivers include new Respite service delivery options: Skilled CNA Respite (unit and day), Skilled RN/LPN Respite (unit and day), and Therapeutic Respite (unit and day), in addition to Unskilled Respite (unit and day). The updated procedure codes and rates are located on HCPF’s Provider Rates and Fees webpage.


This webinar will provide information and answer questions about the new CES and CHRP Respite service delivery options and requirements for service providers. The first hour of the training is intended for Case Management Agencies and the second hour is intended for Providers.

Upcoming PHE Unwind Webinar 

HCPF hosts quarterly webinars on the PHE Unwinding. These webinars are geared toward community partners such as advocacy organizations, providers, and community organizations who may provide other assistance to Health First Colorado or CHP+ members (housing, social services, etc). Presentations and recordings from previous PHE Unwinding webinars are posted in the PHE Resource Center.
