Bridge Holding Table/Bridge Error Message B074

Effective November 20, 2024, the “Selected Benefit Plan” in the Pre Prior Authorization (PPA) in the Bridge will be the primary source of truth for a member’s HCBS Waiver Benefit Plan in the interChange, as long as the member has a current eligibility span from the Colorado Benefits Management System (CBMS) that covers the entire certification period of the PPA. If the HCBS eligible member does not have an HCBS PAR, the interChange will look for a Waiver Span from the CCM (R4) to try to create the member’s benefit plan. Case managers are encouraged to ensure the member’s Program Card in the CCM matches the certification period recorded on PPAs in the Bridge. The interChange processes member benefit plan updates each night at 4 p.m. and the process is completed the same evening. Any active PARs will be evaluated on a nightly basis to determine if the member’s benefit plan needs to be updated in interChange. All benefit plan updates in interChange for LTSS members are sent back to the CCM after the nightly interChange processing. Absent other issues with the PPA, the finalized PAR, and the Benefit Plan will be in place the day after the case manager submits the PPA.


When a case manager selects the “Check Limits” button on the PPA, the Bridge will verify the member’s waiver eligibility. If a member does not have a valid HCBS waiver eligibility span in the interChange, for the certification period on the PPA, the “Check Limits” button will return a B074 error message in the “Messages” tab of the PPA. A B074 message requires that the case manager contact the county to request an updated CBMS HCBS eligibility span. Case managers may view the member’s eligibility span in “Health Coverage” in the CCM system or in the PeakPro system. Once updated, the case manager will resubmit the PPA in the Bridge. B074 will no longer suspend a PAR if the Benefit Plan in interChange does not match the Selected Benefit Plan on the PAR.


Case managers are encouraged to prospectively create PPAs for future certification periods to avoid billing issues. 


HCPF will be addressing existing Prior Authorizations with the B074 error with a conversion process. Only PARs with a PAR ID from the interChange (those that have been submitted and approved at some point in interChange) are subject to this conversion; PPAs that only have a Pre Prior Authorization ID (PPID) (those that suspend before submission and approval in iC) that have the B074 error will not be subject to the conversion process. HCPF is still in discussions regarding solutions; however, the case manager may resubmit these manually. 


Job aides for the CCM that reference B074 are being revised and HCPF will notify case managers when those updates are made.

Rights Modification Training

HCPF will be hosting mandatory training for Case Managers on Rights Modifications, including the implementation of the Rights Modification section in CCM and an overview of the Ongoing Monitoring Guide. Case Management Agencies should be on the lookout for an Operational Memo regarding the updates to the Rights Modification changes and the training requirements.

ARPA Project 5.04: CMA/RAE Collaboration best practices

John Snow Research & Training Institute (JSI) has been working under contract with the Department since February 2023 to conduct a stakeholder-informed process improvement initiative to improve communication, collaboration, and coordination between case management agencies (CMAs) and regional accountable entities (RAEs). We are coming to the end of the contract after 21 months. Following are some of the project’s key accomplishments and recent activities:


  • Overall, JSI conducted 36 key informant interviews (KIIs), facilitated four group forums with RAEs and CMAs, submitted 18 key deliverables, and created a variety of tools and templates to support the implementation of seven priority best practice initiatives.
  • In October JSI conducted and recorded two webinar training sessions. The first training covered the primary functions of a CMA and RAE, and where the two types of organizations’ responsibilities intersect. The second training focused on more details associated with the identified best practices, including implementing process edits for communication and collaboration; considerations for creating and executing memoranda of understanding (MOUs) and data agreements; and utilizing a shared online workspace.   • The Cross Agency Forum, which is composed of RAE and CMA representatives, was established this summer and is currently meeting every month. Some of the topics the Forum has covered include the Forum’s purpose and charter, considerations for data-sharing agreements between CMAs, and RAEs and examples/lessons from successful RAE/CMA collaboration efforts. 


While JSI’s formal engagement ended on October 31, 2024, the Department plans to continue to collaborate with the RAEs and CMAs through the Cross Agency Forum on the implementation of the best practices. For more information, case managers should contact their supervisor or agency’s RAE liaison.

Rapid Reintegration Office Hours/ Work Groups

Beginning Dec. 4, HCPF is hosting office hours/work group sessions so Case Management Agencies can meet with HCPF staff and receive information about Rapid Reintegration. During these times, HCPF staff will provide clarification and answer questions regarding Rapid Reintegration. All interested stakeholders are welcome.  


Rapid Reintegration is a program designed to allow members to make an educated choice about transitioning to the community during the Level of Care assessment (LOC) for Nursing Facility placement. This process will allow the transition process to occur earlier than the current process.​
