Participation in this training session is voluntary, but registration is required for access and attendance. These sessions will be recorded for quality assurance purposes and we recommend attending as many live sessions as your schedule allows to be able to ask questions and hear answers in real time. We look forward to your attendance!

 

Training for New CCM Users

AssureCare introduced a training program for Case Managers who are new to the CCM system. This program is designed to equip Case Managers with the necessary knowledge and skills for their roles.


In order for this training to have its maximum learning benefit, HCPF asks that Case Managers have reviewed recorded demos, short videos, the library of job aids, and have practiced in the TRAIN environment prior to attending these sessions. The New Hire Case Manager Learning Curriculum” document is located in the CCM System Training Google Drive folder to provide a recommended sequence of reviewing the material for an effective learning experience.


Agency leadership, please email Sara Hood after your new staff member has completed the review of the recorded demos, short videos, and library of job aids and is ready to attend training. These training sessions are offered once a month. So, if your staff member has not been able to complete the recommended prerequisites before the scheduled training for that month, we ask them to wait to attend the next monthly training.

Case Managers Week 10/13/24-10/19/24

As you all know, last week was Case Management Appreciation week, and we hope you dedicated time to celebrate -- you deserve it! Not only have you navigated multiple system changes, but you have gone through one of the largest CMA restructures we have ever experienced. We at HCPF appreciate you every single day for the work you do, not only for our members but for the State of Colorado as well. 


Below are some fun facts about Colorado Disability services over the years: 

  • An estimated 5.74 million people in Colorado have a disability, which is about 11.6% of the population. This includes 4.5 million adults and 1.2 million children
  • Colorado is the second-fastest growing state for older adults 
  • Developmental Pathways was one of the very first Case Management Agencies in Colorado, opening its doors in 1964!  


Thank you for all that you have done and continue to do to make Colorado a better place!

Telligen Office Hours & Procedure Code Update

Effective November 1, 2024, Case Managers are no longer required to assign individual procedure codes for IHSS/CDASS/OCC cases submitted in Qualitrac. Only procedure code 99509 will be required for all reviews.

Reminder about Referral Functionality in PEAKPro for Long Term Services and Supports

Reminder: Case Managers will begin to access referrals for Long Term Services and Supports through the PEAKPro system on November 1, 2024.
