Last Chance! Opportunity to Provide Input on Case Management Redesign

As part of the Case Management Redesign initiatives for HCBS waivers, the Department will be hosting two virtual listening sessions for HCBS case management staff to gather information and feedback on current and future definitions of quality case management. Please register for ONE of the following sessions:

11 a.m. - 1 p.m.
Tuesday, Oct. 5, 2021

or

3 - 5 p.m.
Wednesday, Oct. 6, 2021

During each meeting, Department staff will provide a brief presentation on the Department's efforts to redesign the case management system, then case managers will participate in small, randomized group discussions. The feedback received during these listening sessions will be collated into themes and used by the Department in their decision-making on key policy areas for Case Management Redesign.
Case Management Quality Survey - Now in Spanish!

If you are unable to attend one of the dates listed above, the Department has created a survey to gather information and feedback from all stakeholders, including case managers, on current and future definitions of quality case management. This survey will be open to all stakeholders until Oct. 15, 2021. Click here to complete the survey.
 
Recognizing the importance of all member feedback, the Department has translated this survey into Spanish - available here! Please share with all Spanish-speaking members, case managers and providers!
New Assessment Update

The design of the new Assessment is changing from two separate and distinct assessments, the Basic Assessment (required questions only) and the Comprehensive Assessment (required and optional questions), to a single assessment with both the required and optional questions available to the member to answer.

The decision to make this change was made to simplify the design, reduce long-term maintenance of the assessment in the system, improve the ability to navigate through the assessments, as well as to minimize duplicative entry between assessments.

Therefore, we will no longer refer to the Basic and Comprehensive Assessments. We will be revisiting the naming conventions for the new Assessment and will provide an update once those decisions are made.
