This webinar will review the evidence-based Cycle of Engagement Well Visit Planner (COE/WVP) model and tools, which provide a feasible and efficient pathway for clinicians, family specialists and community-based early childhood professionals to partner and engage families to ensure comprehensive preventive and developmental services while meeting performance goals for developmental screening and well visit rates. Empowering Community Health Centers during National Health Center Week!

Community Health Centers play a crucial role in providing accessible, high-quality healthcare to more than 8.8 million children in the United States. Health center staff, including pediatricians, early childhood specialists, community health workers, prenatal care workers, among other team members are essential to promoting the early and lifelong health of children and families. With over 15 health promotion and preventive well-child visits recommended in the first six years of a child's life, health centers are a critical family access point to help set children on a path to thriving long before they enter school.

Building Bridges Between Families, Health Systems, and Communities

The Cycle of Engagement Well Visit Planner (COE WVP) Approach to Care was created in partnership with families, community leaders, and primary care providers to help implement a strengths-based, comprehensive approach to early childhood preventive services. The Well Visit Planner tool:

  • Integrates family-reported screening and priority setting
  • Optimizes time during visits to focus on the family's agenda
  • Tracks population-level needs, priorities, and quality of care

Studies show that over 92% of clinicians and families would recommend this tool to others. The Well Visit Planner's interoperable family data sharing capacity and local population-based aggregator can foster cross-system partnerships essential to promote child health equity and flourishing at a population level!
