On March 4, 2024, the County of San Luis Obispo’s Continuum of Care successfully migrated the Homeless Management Information System (HMIS) to Clarity Human Services. This transition marked a significant step forward for our community in improving and expanding data management efforts strengthening our ability to serve individuals and families experiencing or at-risk of homelessness.


These achievements would not have been possible without the dedication and collaboration of our HMIS community.


Today, we celebrate the incredible progress we’ve made together over the past year!

 

Since going live in Clarity HMIS, we have:

  • Welcomed 4 additional agencies into the system, expanding access to critical resources countywide.
  • Increased HMIS usership by 54% for a total of 244 current HMIS users, empowering more providers with data-driven tools to center client care.
  • Improved care coordination through database enhancements like secure in-app messaging, direct referral to the Community Queue for Permanent Supportive Housing placements, and assessment notifications for case managers.
  • Upgraded reporting capabilities with more than 110+ reports available for users to meet funder needs and provide agencies with deeper insights to support decision-making.
  • Introduced a helpdesk ticketing system averaging 340 messages each month with an average resolution time of less than 6 hours improving user support and response times.
  • More than 244 users completed more than 1,000 courses in the Talent Learning Management Software, increasing training opportunities and expanding the number of courses available for continuous learning.
  • Created an online Knowledge Base with more than 101 articles, offering a centralized hub of resources and guides for HMIS users.
  • Launched San Luis Obispo County’s first public-facing Homelessness Data dashboard thanks to Clarity’s advanced reporting capabilities, which shares client placement and enrollment information.
