AREA AGENCY on AGING DISTRICT 7
Named Winner of National Contest
The Area Agency on Aging District 7 (AAA7) in Ohio was honored recently by the National Association of States United for Aging and Disabilities (NASUAD) through their "Stories from the Field" contest. The AAA7 was named a Second Place winner in the contest and was therefore invited to present at NASUAD's national convention that was recently held in Baltimore, Maryland.
The "Stories from the Field" contest collects examples of business strategies used by disability community-based organizations to sustain or grow their community-based business. The AAA7 has historically served individuals over the age of 60 living with a disability in southern Ohio. After a bidding process in 2014, the AAA7 became a subcontractor for CareSource, a managed care company, to help with care management services for the Ohio Home Care Waiver Program that provides home and community-based services to those under the age of 60 living at home with a disability. The opportunity has allowed the AAA7 to branch out to more populations, now including those from birth to age 59, as well as expanded geographic reach with three additional counties to provide these specific services in, for a total of 13.

Open Enrollment for Medicare
Ends
December 7th -
AAA7 Available for Assistance and Help
Are you a Medicare beneficiary? Are you aware that open enrollment for
some Medicare benefits ends December 7th? Open enrollment is the
time of the year when changes can be made in the plans and providers
you've chosen for your Medicare Part D prescription drug coverage
and/or Medicare Advantage option. If you wish to make changes,
you can only do so between October 15th and December 7th. Open enrollment is also a good time to run drug comparisons to make sure the Medicare Part D plan you are on is still your best option as Medicare Part D plans do change from year to year.

COMMUNITY LEADERS NEEDED for
Chronic Disease & Diabetes Self-Management Programs
Interested in being trained to help your community better manage their chronic disease conditions? The Area Agency on Aging District 7 (AAA7) is looking for community leaders to assist with facilitating its Chronic Disease and Diabetes Self-Management Programs. Community leaders/coaches will continue to support the classes in their individual communities after they complete training through the AAA7. An upcoming training is scheduled in Portsmouth (Scioto County) beginning on November 6th for those individuals who are interested. The AAA7 covers ten counties in southern Ohio, including Adams, Brown, Gallia, Highland, Jackson, Lawrence, Pike, Ross, Scioto and Vinton.
Self-management programs for people with chronic conditions are designed to help those with chronic diseases, such as high blood pressure, asthma, arthritis and diabetes, learn to manage their conditions and take control of their health. The Chronic Disease Self-Management Program (CDSMP) and Diabetes Self-Management Program (DSMP) was created at Stanford University and are proven community initiatives that provide health benefits and promote disease prevention.

JPMorgan Chase & Co. (JPMC) has announced the launch of AdvancingCities, a $500 million, five-year initiative to advance lasting solutions that will lead to more people connected to jobs with a career pathway and living with financial security, more small business owners growing and hiring, and economically vibrant neighborhoods.
The firm will invest in cities where conditions exist to help those who have not benefited from economic growth. This includes demonstrated, strong collaboration across the public and private sectors on solutions that create opportunity for people at risk of being left out of economic growth. Proposals must incorporate at least two areas of focus within JPMorgan Chase's
Model for Impact
.
Solutions will help break down silos between local programs, strengthen underlying systems and advance holistic solutions to create more widely-shared prosperity.
Cities interested in applying for the AdvancingCities Challenge should visit www.jpmorganchase.com/advancingcities. The RFP went live and closes on November 30, 2018. Winners will be announced in the spring of 2019.
