In November of this year, we were devastated by the loss of our first founder and a legend in the industry, Roy Christensen. Roy often spoke of his crowning achievement as being establishing an organization where decision making and leadership is at a local level, individualized for every facility, and as close to patient care as possible. He helped to lead the way in the establishment of our Core Values: CAPLICO, and encouraged these values to be living and breathing principles that guide our decision making and how we treat one another. Today, our leaders stand on the shoulders of giants like Roy.
As we prepare to ring in a new year, the articles in this special “Leadership Highlights” edition of the FlagPOST are dedicated to our beloved Roy.

When Lito Ortiz first started with Southland Care Center in Downey, CA in 2003, he was only 21 years old. He was a foreign-trained physical therapist, and started as a Rehab Tech. Lito was beloved by patients and staff, and in 2005 he pursued his Physical Therapy Assistant (PTA) degree. He continued to treat patients at Southland and neighboring Brookfield, The Orchard, and Whittier Hills when he decided to pursue his Physical Therapy License in 2006. A few years later, Lito caught the leadership vibe and became DOR at Brookfield Healthcare Center in 2010, where he transformed the department and entire facility to a deficiency-free two-time Flag Flying juggernaut. In 2015 Lito and his family headed west to Texas to invigorate and inspire culture at Golden Acres. He helped to move the therapy department front and center and got the flywheel moving before he moved back to California to become a Therapy Resource. In 2018 Lito was awarded CTO recognition, and that same year he furthered his schooling and earned his Doctorate of Physical Therapy, all while serving in both a DOR and Resource role. If you ever get an opportunity to meet Lito in person, there is no doubt that you will be inspired by his passion, creativity, entrepreneurial spirit, and above all, his genuine kindness. He truly represents “Building Therapy Leaders” in every possible way!

Amber Thompson, PTA, LNHA, Market Leader Keystone North Central Market (pictured on right with daughter Taylor).
“Leadership is the ability to translate a vision into a reality” (Warren Bennis). This quote has fueled the growth of Therapy Leader, now CEO, Amber Thompson for the last 17 years. Amber started in healthcare 17 years ago as a PTA. She moved into a role of Director of Rehab, then moved into operations as Executive Director, and most recently she is serving as Market Lead for Keystone North Central supporting other Executive Directors/Resources. Amber shares that her favorite thing about our culture is that we are a family. We support each other during tough times, pick each other up when we get knocked down, and hold each other accountable when we start to slide. Through all of the challenges and successes we encounter as a team, we love each other unconditionally and are very supportive. As one grows with leadership roles, it’s important to remain true to your values and be passionate in what you do. Your passion will inspire others to be their best. Be humble enough to ask for help and NEVER stop learning. Amber shares that she is a big advocate for therapy taking ownership within the facility, and explains that as a DOR, one might not realize that opportunity. The therapy department truly has so much to contribute to the cultural, clinical, and financial success of an operation. When truly working as an interdisciplinary team, you diminish silos, build trust, grow leaders, improve culture, and have better clinical outcomes. These results lead to a healthy operation which creates the environment for happy and healthy residents.

Cate Dyer, (pictured right with Jackie Eaton) is the ADOR at The Healthcare Resort of Topeka. Cate started as a COTA in June 2018 at The Healthcare Resort of Topeka. Cate’s passion for our residents and patients was evident in her willingness to take on challenging Plans of Care, her interest in developing their Dementia and Contracture Management programs, and her eagerness to learn the administrative side of the department. Her yearning to increase her circle of influence was driven by her admiration for Jackie Eaton, TPM, and her desire to assist with the daily administrative duties to be the department back-up when Jackie was out of the clinic. When the DORiTO (Director of Rehab in Training Opportunity) program rolled out, Cate expressed interest, and Jackie recognized Cate’s leadership potential and nominated her to participate in the program. Cate is now Jackie’s ADOR and embraces her leadership role on a daily basis and is able to effectively run the department when Jackie is out. Jackie continues to be dedicated to Cate’s leadership growth and gives her opportunities to grow as they arise.

Abi O’Keefe, DOR at The Healthcare Resort of KCK. Abi started in August 2015 as an OTR when The Healthcare Resort of Kansas City opened. Abi excelled at her role as an OT and quickly became the lead OT for the department. Abi demonstrated exceptional leadership qualities, and her DOR asked and trained Abi to be the department ADOR. Abi embraced her new role and actively sought opportunities to learn more of the administrative role within the therapy department and outside the therapy department and rapidly developed as a leader. In March 2017, Abi became the DOR when her predecessor moved on and has continued to grow herself and other leaders within her department; she has an ADOR, multiple program champions, established support groups and specialty programs, and she assists in other markets with acquisitions and helps drive the Cluster Model within Emerald Market.

Ali Vandeloo, DOR at Rock Creek of Ottawa. Ali started in 2017 at Park Avenue in Arizona and in 2018 moved to the Kansas City area. When Ali shared with her DOR in Arizona her plan to move closer to home, the DOR immediately contacted the local therapy resource because she knew Ali embodied our vision and culture and had to stay within the organization. Ali joined our newly acquired facility, Rock Creek of Ottawa, on Nov. 1, 2018 as the OT and soon became the lead OT. Her influence expanded beyond the OT department, and she assisted in training and orientation of all new therapists within the department. Ali implemented the Abilities Care Approach Program, Contracture Management and various other programs at Rock Creek. Her exceptional leadership skills were rewarded when she became the DOR after her predecessor became the facility ED.

Stephanie Anderson, Executive Director at Rock Creek of Ottawa. Stephanie started as a new DOR when we acquired the local SNF in Ottawa in November 2018. Her previous therapy experience was in the Home Health and Outpatient settings, but she had a desire to make the local SNF a place of pride for her community and joined the leadership team at Rock Creek of Ottawa. Stephanie’s leadership skills helped establish processes, systems and programs; drive culture throughout the entire facility; and set a standard of excellence during a time of uncertainty and change. The steep learning curve of the SNF setting was no match for Stephanie, and her desire to grow only enhanced with each experience to the point she decided to take the plunge to become an Executive Director within the organization. Her dedication to her development resulted in her becoming the Executive Director of Rock Creek, and shortly afterward, she took on the role of the Cluster Lead, providing her support and expertise throughout the market.

Beth Munford, Executive Director at The Healthcare Resort of Olathe. Beth started her journey as the Director of Rehab in April 2019. Under her leadership, she implemented the Abilities Care Approach program, enhanced the Rock Steady Boxing outpatient program, and implemented multiple LTC programs, taking HCR of Olathe to new heights never thought possible before! Her desire to grow programs was not limited to therapy; she helped boost clinical processes and systems to help the facility achieve a 5-Star rating in QMs and overall while she was the DOR. Through her outstanding leadership, Beth received recognition as the very first Chief Therapy Officer for the Emerald Market! In late 2020, she embarked on her journey to become an Executive Director and is currently the Executive Director at HCR of Olathe. She is a true testament to leadership development!

Danielle Banman, Emerald Therapy Resource. Danielle started as the DOR at The Healthcare Resort of Leawood in April 2019. She joined us with many years’ experience as a proven leader and shared her passion for LTC programming, creating a synergy of LTC energy throughout the therapy team. Her hunger to continue to serve the LTC population inspired her to learn more about the Abilities Care Approach program and eventually became a TEACHA for the Market. She is now sharing her passion, knowledge and skills as the Emerald Therapy Resource, building more great leaders.

Carly Kenney began with our facility when she wasn’t actually intending to. She came to Coral Desert Rehab for a job interview to be a rehab tech. She didn’t realize that we shared a parking lot with a competitor outpatient therapy company where she had the actual appointment. As the rehab director at the time, I was looking for a rehab tech anyway, so I went ahead with the interview and offered her the job on the spot, and she accepted. We’re all so blessed that she did!
After spending a few years making our Therapy department shine by taking on every kind of responsibility we could throw at her, Carly saw an opportunity for growth within Coral Desert in the admissions and case management department. Read On...

Shelly Rogers started as a floor CNA in 2013 for one year at Catalina Care Center in Tucson. Due to her amazing personality and high level of ambition and professionalism, she was promoted to Social Services Assistant and excelled in that position for one year. At that time, the SS director resigned, and Shelly was given an opportunity to manage Social Services as Director. She has been our amazing SS Director since 2015 and now has a full-time assistant of her own, and she is frequently used as a trainer for other buildings’ new SS employees. She’s an amazing asset to our team!

Kyle Martin. Kyle began working at Legend Oaks-New Braunfels as a physical therapist at the date of its opening in 2015. He then was able to grow into the Assistant Director of Rehab role, followed by the Director of Rehab, where he became CTO, and later to a Therapy Resource. After working as a Therapy Resource, he became an Administrator In Training at Legend Oaks-New Braunfels. He has now successfully been the ED at Kirkwood Manor for the past two years and is serving a cluster leader in his market.

Albert Lejia. Albert began working at Legend Oaks-New Braunfels when it opened in 2015. He started as an incredibly motivated, caring and driven CNA. He became the lead CNA and became an educator to other Nursing staff regarding documentation and how to appropriately assess patients’ ADLs. He was promoted to the Rehab Tech and Restorative Aide for the Therapy department, where he was able to provide outstanding care and assistance to the Therapy department. Following being a rehab tech, he was promoted to Central Supply. Albert is now the Maintenance Director for Legend Oaks-New Braunfels and continues to shine and demonstrate potential growth as one of most outstanding employees at the facility.

Camrin Nettey is our new DOR at The Healthcare Resort of Leawood! Camrin has led the speech therapy team by implementing Heart Math, Speak Out! and Ampcare. Shortly after completing the DORiTO program, Camrin transitioned into the DOR role and continues to lead the entire team to provide outstanding therapy services.

Nikki Wiegel joined Riverbend Post Acute Rehab Center as a PTA in 2019. She has been the Therapy Program Manager for over a year now and has been instrumental in leading therapy and her IDT to have a successful dementia care unit using the Abilities Care Approach as well as providing exceptional care to their LTC and ALF residents. We are so lucky to have this amazing leader here in Emerald!

Welcome Mary Combs, PTA, New Therapy Program Manager at La Canada Care Center in Tucson, AZ! Mary is a native Tucsonian and a great example of leadership growth and development. Mary has been practicing as a PTA for five years. Within Ensign-affiliated facilities, she started as a PTA at Catalina Care Center. She then became the Case Manager at La Canada Care Center, showing that rehab-trained individuals can function in many roles! Mary attended the DORiTO program and is now the new Therapy Program Manager at La Canada.
Among many goals, Mary hopes to establish a solid PT wound-care program in conjunction with Nursing. This is particularly impactful, as La Canada specializes in the SMIP (Substance Misuse Inpatient Program), with many wound-care needs in this population. Maximum resolution of wounds upon discharge is a huge indicator for success with these folks. Mary is a mom to two children: Grace, 16 months; and Gus, 2 months. In her spare time, she’s managed to join a women’s soccer league! Welcome, Mary, we are excited to have you.

Nathan Nakashima, BOM, started at Rainier Rehabilitation in June 2014 in the Therapy department. He was the therapy tech from 2014 to 2018 at Rainier and also helped other buildings by filling in when they didn’t have a tech and helped train new therapy techs throughout his time there. He later moved up into the Business office as the ABOM from September 2018 to August 2021 because of his understanding of insurances and RUG levels throughout his training and advancement in general. Nathan learned a lot about the business side of skilled nursing facilities. Nathan said, “August of 2021 was my first official month as the BOM at one of our new acquisitions at the Oaks of Lakewood, where I am still currently at. My resource Tricia, Mira, other upper management at Rainier and family all pushed me and helped me advance to where I am currently at today, and I am forever grateful for the opportunity.”

Roohi Kapoor, PT, DPT, is the current Operations Manager at Copperfield Healthcare and Rehab. Prior to that, she ran a powerhouse program as the DOR at Mason Creek. As a DOR, she hesitantly stepped into that role about three years ago and quickly shifted the clinical and operational metrics upwards — so much so that she earned the coveted honor of Chief Therapy Officer in early 2021. Ebony Prater first joined our organization as a TheraTooper. She came to Keystone and never left! She was introduced to different facilities and became interested in a Director of Rehab position at our Northeast Rehabilitation and Healthcare Center and grew the Long Term Care Program there to new heights. With every visit, the therapy gym was always full of ladies and gentlemen having a great time and getting stronger. Ebony was first in line to introduce the Abilities Care Approach as well as the Drumming Program, bringing both function and enjoyment to her team and lovely residents. In pursuit of further growth, she joined the AIT program and is now the Operations Manager at one of our new acquisitions, Sedona Trace. She has brought her LTC Programming passion to Sedona Trace, and we have been able to bring so many moments of love to more people in the Austin area! We are so proud of Ebony and the Sedona Trace gang!

Shaun Baldwin started his SLP career as a CFY/SLP at our San Marcos Rehabilitation facility. He obtained his SLP/CCC and became an outstanding clinician. He then moved up to the Director of Rehab role at San Marcos Rehabilitation. He was able to grow this program and became known as our PASRR King. He became such an advocate for our PASRR residents and really grew our PASRR programming in Keystone. He was our go-to for this program and really worked closely with our MDS and AR partners. He was able to figure out how to bill and give great therapy for these ladies and gentlemen. I am so very proud and thankful for his diligence with this population and all his patients he has served. Shaun then decided to grow as a leader and is now the Operations Manager for one of our new acquisitions, Cedar Pointe Health and Wellness Center in Cedar Park, Texas. I love how we are growing leaders in our company!

Andy Cisneros (pictured with wife Marissa) has been a PTA for 21 years. He was a DOR that transitioned into working with our buildings with the Legends acquisitions in 2015. At that time, he was at Legends West San Antonio, where he remained until 2019. He then transitioned into a new acquisition building, Westover Hills. He grew the long-term care programming in this facility, was a cultural leader, and ended up leading the activities within the facility. Andy became CTO while at Westover Hills, since November 2019. In November 2021, Andy transitioned into the therapy recruiter position for the growing north/south central and Borderstone areas. Andy has been actively working with HR in the area, attending career fairs and recruiting events to find the best therapists for the facilities he is supporting. Welcome, Andy!

After graduating college in 2010, Russell Sells found himself in a tough job market in the greater Sacramento area. Unable to find full-time work as someone without much job experience, he settled for multiple part-time positions while still searching for full-time work. After a conversation with a friend from college who was living in the Olympia, Washington, area, he decided to apply for work up in Washington.
Russell says: “One of the first positions I applied for was a Therapy Assistant position (thinking it was a Rehab Tech job). The first person I spoke with was Mira Waszak. She informed me they were looking for licensed assistants but also offered to put me in contact with someone who needed a Rehab Tech for a new building acquisition in Olympia. Just wanted to take a minute to brag about my amazing ADOR Lyda Latagliata 😊. Lyda has been working with me for a few years now and has always been hesitant about taking a student due to a less than savory experience at a prior place of employment. When we had a change in staffing around this time last year (aka our OT Melissa took a DOR position at Coronado), Lyda stepped up to take on an OT student we had already committed to. Two things now: 1. Lyda now has fully embraced her role as a clinical instructor and has been an amazing partner to our schools out there. Her calendar is booked with practically one student after another. 2. (And my main reason for sending this out today) Lyda was nominated for and selected as A.T. Still University’s OT Clinical Instructor of the year award! I truly couldn’t be prouder and wanted to share this with all of you!

As many of you may be aware, Keystone Ridge spent the last couple of years on the Special Focus Facility (SFF). Not only did this facility move off the SFF list this past year, but it also just recently passed its annual survey with flying colors! There are many individuals who have contributed to this remarkable change, but I want to particularly highlight Dawn Young, COO, and Tara Meyerpeter, DOR.
Dawn (pictured left) came to Keystone Ridge when it was struggling and has navigated a number of surveys as an SFF. Now, she's seen the facility through its first survey as a non-SFF, resulting in just four tags. The amount of time that she has poured into Keystone Ridge has been amazing, along with the incredible effort she contributes as a cluster partner to the NE Market. Her COO recognition she received this year was well-deserved.

Tara Meyerpeter, DOR joined us at a similar time to when Dawn did as DON. I have seen Tara grow so much over these past couple of years as a Therapy leader but also as a strong IDT partner. She has taken what was a small Therapy team and added two full-time speech therapists, one full-time physical therapy assistant, and one full-time occupational therapist. She's also developed an outpatient program on that campus that is excelling beyond anything we have seen in the past. In what is a very challenging payor source environment with a lot of Medicaid-only payors for therapy, she has still managed to consistently double if not triple per month in the care they provide compared to prior years. Tara continues to excel in her growth as a cluster partner and is constantly pushing her team to expand clinical programs and provide the highest quality of life we can to our residents.

Marie was celebrated with love and gratitude on November 19, 2021, for her 12 years of extraordinary service as a physical therapist. Marie retired last week. The staff and residents are saddened that they won’t see her warm, smiling face every day. However, Marie is remaining at Osborn PRN, which is a result of her unparalleled dedication. Thank you, Marie, for being eternally upbeat, raising the bar to your profession and positively impacting the lives of the residents at Osborn. We wish you joy, health and happiness with your retirement.

An Inspiring Story from Shea PARC

Shea PARC of Scottsdale, Arizona, reached an all-time skilled census high of 49 last month! They overcame staffing challenges and continued to provide the highest quality of care to all of their residents. It was during this arduous month that they had yet another amazing success story. They were able to take a LTC resident out of the building for the first time in two years to celebrate her birthday. Prior to therapy, family and staff would have to lean into this resident to hear her speak, often asking her to repeat herself.
The speech therapist worked on breath support and voice training. The resident gained enough strength to no longer require supplemental oxygen. She now holds prolonged conversation with improved volume and intelligibility. This resident had a terrific time at lunch! She even ordered her own meal and dessert and was able to blow out her candle! Way to go, Shea PARC team!

“Driving on the Healing Pass”

Today, we celebrated with one of our outpatients who successfully drove herself to the facility, something she could have not done in the last six months. Our Occupational Therapy Assistant who mainly has her for sessions cried in joy with our patient, celebrating her achievement. Words cannot describe the emotion that was seen as the whole rehab department clapped in joy.
Our patient June has been coming to our facility for Occupational Therapy Outpatient Services to work on both of her hands to regain function and take her life back into control. Prior to coming, she was one of our in-person residents who then discharged out of our facility and became our very first outpatient. During her stay, she developed a strong bond with the nursing staff, housekeepers, and our amazing Occupational Therapy Assistant Camille Navata and Occupational Therapist Joe Zaldua. She regained her ability to do her daily routines while wearing a soft cast that covered all the way up to her PIP joints of her fingers. Once the soft cast was removed and she was cleared for discharge, she declined to attend other outpatient clinics in the area that were recommended by her MD and decided to work with Camille and Joe once again for our outpatient services. About the FL Spartan Beast:
The ultimate Spartan test; a soul-crushing half marathon with 35 obstacles. You’ll rarely find a Beast on flat ground, so your legs will burn with brutal ascents and descents. The Florida Beast is notorious around the world for its difficulty due to two entire miles completely being in the swamp with up to chest-high muddy water.
Amber and Jon have done several Spartans together, including a Beast in the Rocky Mountains. We find that having a big fit test planned out like a Spartan Beast creates an opportunity of accountability and ownership for us to maintain our fitness, nutrition and overall healthy lifestyle. In fact, for 2022, we have spaced out three separate Spartans around the USA to train for, and we hope to recruit other like-minded Spartans to join us as we make 2022 our fittest year ever!

Embracing the Spirit of Wellness

In today’s environment, it is easy to exhaust our energy by worrying about and taking care of others. However, if we don’t take care of ourselves first, this can be a near impossible task. In order to embrace the spirit of team and wellness, Two Clusters (eight facilities) recently promoted a self-care/wellness challenge in the Bandera West Market. Therapists were challenged to choose up to three self-care activities and the frequency they would commit to performing each task per month over a three-month period. Each therapist was given a designed calendar to document their activity. Some teams showcased a Wellness Wall, hanging everyone’s calendar to help hold each other accountable.
Prizes were awarded to individuals who completed their self-paced challenge, and an extra award was given to the three teams who had 100% participation and follow-through, as well as being awarded the gift of Wellness.

Congratulations to Bella Vita Health and Rehab, North Mountain Medical Rehab Center, and Granite Creek Health and Rehab for winning the Wellness challenge. To continue the Spirit of wellness in 2022, each facility will have the challenge of total team participation along with choosing another department within the facility to spread the team approach of helping each other BE WELL!

A huge thank-you to everyone who submitted information on how vital signs in therapy are being integrated at their facilities. We had amazing entries from across the organization. We were blown away by how vital signs have become such an integral piece of therapy services. We read the most clinically comprehensive documentation and saw how decisions around care were driven by vital signs during therapy sessions. We also were impressed on how vital signs data collected in therapy was used for IDT communication to ensure the very best collaborative care.
Overall we want to thank everyone – whether you submitted something in or not – for your focus on providing the best care using all resources, including integrating vital signs.
