Tobacco Free Florida:
Automated Electronic Referral Process
Tobacco Free Florida has developed a new, more efficient way to incorporate electronic referrals into organizations' systems by creating a link to use an Automated Referral Form. This automated referral form includes the same fields as the fax referral form most providers currently use. Acquiring the form is easy:

Go to https://tobaccofreeflorida.com/healthcare-providers/ to request a unique link to refer your patients electronically. This allows for the referral form to be preloaded with your contact information, creating a unique webpage for your organization to submit referrals.

Once the request for a link is received, a representative from the University of Florida AHEC (noreply@tffact.com) will respond within 2 business days with a unique link for your organization to use. After that, when the provider completes an automated referral form on behalf of a patient who wants to quit (within the next 30 days), the referral is automatically sent to AHEC for group quit or Optum for phone quit, depending on the patient’s selection.

Florida State Health Assessment Survey
The health councils in Florida are working with a diverse array of partners to develop the 2021 Florida State Health Assessment (FSHA) and the 2022-2026 Florida State Health Improvement Plan (FSHIP).

The resident survey is now available through August 31, 2021. The survey is available in English, Spanish, and Creole. The survey results will help guide the FSHA/FSHIP process. Please assist by:
  1. Taking the survey
  2. Sharing the survey with your network
  3. Getting family, friends, and clients to take the survey as well

If you are a provider (someone who provides, coordinates, and/or manages health care for the residents of Florida) you have the option to take the SHA Provider Survey. This survey will be available through August 31, 2021. Please feel free to share this link with other provides.

If you have any questions, please contact the survey coordinator at info@statehealthassessment.org.
