Advancing HRSN Data Integration
Collaborating for Better Patient Care
| | We’re excited to share the growing momentum in Health-Related Social Needs (HRSN) data sharing across the region. Together, with our health and care partners, Social Care Networks (SCNs), and technology collaborators, we're driving smarter patient navigation and more timely referrals statewide. | |
The milestones below reflect the deep commitment of our partners and expansion of our shared infrastructure to support the 1115 Medicaid Waiver initiative.
HRSN Data Exchange in our Region
To date, our network has achieved:
| |
- 28,907 HRSN Screenings
- 11,158 Comprehensive Assessments
| |
- 18,692 Open Referrals
- 23,146 Confirmed Services
| | |
EHR Integration for HRSN Screenings
To improve screening workflows, we’ve made substantial progress integrating HRSN screening data directly from electronic health records (EHRs), enabling seamless data exchange between clinical and community settings.
We’re now actively receiving data from:
...with additional integrations on the horizon!
| | |
What’s New: Healthy Alliance App Integration
A key development currently in progress is our collaboration with Healthy Alliance, a SCN Lead Entity under New York’s 1115 Waiver. Together, we are working to fully integrate their screening processing into our data exchange ecosystem. This HRSN data exchange method will soon offer providers across 24 counties, including Central NY, the Capital District, and the North Country, an alternative for sharing HRSN data.
| | Through this partnership, providers will be able to transmit screening data to their SCN, and we'll support the immediate connection into the SHIN-NY Data Lake. This advancement will enable frictionless data exchange, to increase access to essential care and services for Medicaid Members across the region. | | | |
