Expanding Our Reach in the
Rochester and Finger Lakes Region

In August, the Rochester RHIO announced a partnership with HealtheConnections to provide HIE services to its participants. This is an exciting step not only for our regional networks, but for the broader statewide effort to streamline Statewide Health Information Network for New York (SHIN-NY) infrastructure, share services, reduce duplication, and make the best use of healthcare resources.

HealtheConnections has maintained relationships across the Rochester and Finger Lakes regions for many years. Patients regularly receive care across regional boundaries, making this a natural partnership that strengthens connections between organizations in each community.

The technical transition is under way and participating organizations will begin being onboarded to HealtheConnections starting in December. To learn more, visit our Rochester RHIO transition webpage.

Connecting Data to Community Health

Public Health and Community Insights

Expanding Health-Related Social Needs (HRSN) Data Exchange

As New York continues to advance social care initiatives through the 1115 Medicaid Waiver, HealtheConnections now supports 30+ off-platform HRSN screening connections, helping providers securely share HRSN data through existing workflows.

We’re also pleased to welcome Forward Leading IPA (FLIPA) as another Social Care Network (SCN) working with HealtheConnections to exchange HRSN data. Together, these efforts help make social needs information part of a more complete patient record and support stronger coordination across healthcare and community services.

See the impact: Learn how Social Care Networks are connecting New Yorkers with services and support. Watch now.

Piloting Timely Public Health Birth Alerts

Using the same infrastructure that powers Statewide Encounter Alerts (SEAS), HealtheConnections is piloting birth-related alerts for local health departments. Leveraging statewide hospital data, these alerts are generated based on county-level populations rather than defined patient rosters, helping health departments reach new mothers sooner.

  • Statewide reach: Alerts regardless of where care occurs
  • Person-level information: Identifies individuals who may benefit from outreach
  • Timely delivery: Information provided twice weekly to support earlier engagement

Community Health Insights, Powered by HIE Data

HealtheConnections’ Community Health Dashboard is now live in the HIE Portal for public health users. Powered by HIE data, the dashboard provides community-level insights to help identify trends, understand needs, and inform public health action.

The dashboard is expected to become available to all HIE Portal users in the coming months. Stay tuned for updates.
