Patient Centered Medical Home Update
|
|
During this week's PCMH conference call, VIM staff reviewed the clinic's
Gap Analysis Worksheet
with the Americares team. The group reviewed the clinic's Quality Improvement infrastructure, the
six domains
of healthcare quality and the NCQA standards for QI.
The next assignment for VIM to complete is a
PDSA Worksheet
. This Plan-Do-Study-Act Worksheet will assist the clinic in documenting a test of change for the
implementation
of new programs as the PCMH certification process progresses.

VIM Attends Geisinger Commonwealth School of Medicine's Community Service Fair
|

On Thursday, September 13th,

, Development Manager, and
Chelsea Collins
, Social Care Coordinator, attended Geisinger Commonwealth School of Medicine's Community Service Fair at the campus in Scranton. The students were
very interested
to learn about all of the
services the clinic
provides to eligible patients and they look forward to volunteering in the future.

United Way's Annual Kick-off Dinner
at Mohegan Sun Casino. The evening's keynote speaker was
Father Greg Boyle
, founder of Homeboy Industries in Los Angeles, the largest gang-intervention and
rehabilitation program
in the world. His powerful presentation touched everyone in attendance.

Social Care Coordinator Attends National Association of Social Workers Conference
|
|
On Friday, September 14th,
Chelsea Collins
attended the
Pennsylvania Chapter of the National Association of Social Worker’s
annual conference at Kalahari Conference Center. Chelsea participated in the
“Integrated Behavioral Health Certification”
training. The training was a 7-hour intensive workshop and focused on social workers’ roles in helping integrate and expand behavioral health services in a primary care physician offices. After
successfully completing
the training, Chelsea received a certificate for providing integrated healthcare services in a primary care setting.
She also ran into
Dr. Weinman
during the lunch break which was a happy and unexpected surprise! Dr. Weinman was attending the conference with his wife, who is also a social worker.

VIM's Attends Race for the Cure

, hosted a grantee's table at the Susan G. Komen Race for a Cure in downtown Scranton.

VIM Provides Dental Screenings to the Children's Development Council of NEPA
|
|
On Thursday, September 13th,

, VIM's Dental Hygienist and
Dr. Mahima Singh
, VIM's Dental Director, provided dental screenings at the
Children's Development Council of NEPA's
Nesbitt Center in Kingston. One of the children was afraid of seeing the dentist so he decided to make Dr. Singh a
firefighter
for the day!

Masters of Social Work
program at the clinic. She will be observing Chelsea, seeing patients and assisting with the
Health Equity Program

Della Huffman
, a Geisinger Family Medicine Resident, spent time seeing patients in
conjunction with Dr. Scott
at the clinic this week. She was happy to be assigned to VIM because she gained
experience treating patients
in a free clinic setting.
|
|
Recently, a student from Luzerne County Community College's
Dental Hygiene Program
performed a deep cleaning on a patient that hadn't seen a dentist in over ten years. Often times, patients that come to VIM's dental clinic have
numerous caries
and require 2-4 appointments to get
out of pain
and on a preventative care schedule.
During the appointment, the student encountered
unusually thick
and large calculus that had accumulated behind the patient's lower anterior teeth. Due to the
extreme circumstances
, the student notified Dr. Singh before beginning the patient's procedure.
Because of the extent of the
calculus build-up
and the amount of time the deep cleaning takes, only one quadrant of the mouth was able to be completed. An
additional three appointments
will be needed to complete the patient's scaling and root planning procedure.
