Community Care HUB to celebrate 5th anniversary

This August, the Community Care HUB, an initiative of Center for Population Health, is celebrating its 5th anniversary.

In September of 2020, the HUB was launched for residents in Cambria and Somerset counties.

A HUB is an organized, outcome focused, network of Care Coordination Agencies (CCAs) who hire and train Community Health Workers (CHWs) who help connect at-risk individuals to needed services.

The HUB's CCA partner agencies are Alleghenies United Cerebral Palsy, Beginnings, Inc., and The Women's Help Center.

Preparedness and Resiliency Partnership launches

The Cambria and Somerset Counties Preparedness and Resiliency Partnership has been newly created to build preparedness and resiliency within our communities. This collaborative group creates the ability for the two counties to withstand, adapt to, and recover from challenges, such as natural disasters, economic difficulties, pandemics, or other crises that would arise. The partnership includes multiple key community stakeholders (e.g., government agencies, local organizations, businesses, and residents) all working together to improve our collective ability to prepare for, respond to, and recover from disasters or challenges in ways that benefit all members of the communities that we serve.  

The Partnership meets the 1^st Thursday of each month at 10AM, online, to accomplish its intended objectives.  If you are interested in learning more or joining the meetings, please email jmcmillan@centerforpophealth.org.

CPH Local Food Systems Coordinator has busy summer

This summer has been full of activity for Center for Population Health’s Local Food Systems Coordinator Nan McNinney. She visited with food advocates, helped Cambria County residents sign up for SNAP benefits, helped set up community gardens and so much more.

CPH Communications Coordinator tables at Farmers Market

This month, CPH Communications Coordinator Allie Byers setup a table at the Johnstown Farmers Market.

Allie provided useful resource information to those in attendance.

CPH sponsored Health & Human Services Fair at a Mill Rats game

The Center for Population Health held its 4th Annual Health & Human Services Fair at the July 19 Johnstown Mill Rats game.

11 health and human services agencies set up tables at the event with resource information and even giveaways.

It was a great evening to learn more about local resources and watch a baseball game.

Health & Human Services Fair to be held at the Treehouse

The Center for Population Health is working with Small Town Hope/The Treehouse to hold a Health & Human Services Fair on August 14.

Back to School Health & Human Services Fair to be held at Westmont Hilltop

On Thursday, September 4, the Center for Population Health and Westmont Hilltop School District will be holding its annual Back to School Health & Social Services Fair.

CPH tables at Families R Us event

CPH Executive Director Jeannine McMillan tabled at the Families R Us Community Event in the Oakhurst Community on July 26.

It was a great opportunity to provide resource information to families.
