2025 Community Health Needs Assessment will be launching soon

The Conemaugh Health System, 1889 Foundation, United Way of the Southern Alleghenies, and Center for Population Health, along with additional community partners, are getting ready to launch the 2025 Community Health Needs Assessment (CHNA) for Cambria and Somerset Counties.

A CHNA is a systematic collection, analysis, and dissemination of information about the health of the community. Its role is to identify factors that affect the health of a population and determine the availability of resources within the community to adequately address those factors.

The CHNA will help to support many community wellness and redevelopment efforts in our region already underway, and provide the supporting data needed for decision making across these multiple initiatives.

In the coming weeks, the survey will be shared via email, hard copy, and postal mail to obtain your feedback… stay tuned!

A central focus of the conference was workforce development, highlighting strategies to grow, support, sustain Community Health Workers (CHWs), and strengthen Community Health Networks.
