Big changes happened at the Center for Population Health in 2023. CPH officially became an independent 501(c)(3) nonprofit organization.


In 2023, the Center for Population Health (CPH) worked hard to continue serving the residents of Cambria and Somerset counties.

Governor Josh Shapiro visits Johnstown

CPH Executive Director Jeannine McMillan and 1889 Foundation President Sue Mann attended a press conference January 22 held by Governor Josh Shapiro and state legislators at the Greater Johnstown Community YMCA.


Positive enhancements to the Commonwealth's Property Tax/Rent Rebate and Childcare Tax Credit programs were announced, which are designed to financially assist seniors and families.

Community Care HUB collaborating with AmeriHealth Caritas Pennsylvania

AmeriHealth Caritas Pennsylvania and Center for Population Health (CPH) are collaborating to improve health outcomes and reduce health disparities for AmeriHealth Caritas Pennsylvania members living in Cambria and Somerset counties. 


The Community Care HUB, implemented by the Center for Population Health, offers community-based care coordination for a variety of at-risk individuals. 


AmeriHealth Caritas Pennsylvania members who are pregnant or parenting children up to 18 months of age are referred for this service and connected with a Community Health Worker (CHW) who identifies and addresses their health and human service needs. Through face-to-face interactions, the CHW strives to improve health outcomes by linking eligible members to various community resources that reduce both medical and social barriers to care, such as employment, housing, food access, and transportation needs.

COVID-19 Task Force funding first responders, healthcare personnel retreat

The Cambria-Somerset COVID-19 Task Force is sponsoring an Emergency Services Wellness Retreat for EMS, fire, police, 9-1-1, and healthcare personnel. It's an opportunity for first responders and healthcare personnel to build relationships and learn coping mechanisms from individuals who have faced the same challenges.


The retreat will be held at Outdoor Odyssey. Five separate, week-long events will be held throughout 2024.

CPH was able to connect the Johnstown Housing Authority with Giant Eagle Pharmacy to provide on-location RSV, shingles, COVID-19, and flu vaccines to the residents in Town House Tower, Connor Tower, and Vine Street Tower in January.
