2025 Community Health Needs Assessment finalized

The Conemaugh Health System, The United Way of the Southern Alleghenies, 1889 Foundation, and the Center for Population Health, along with community partners, have finalized the 2025 Community Health Needs Assessment (CHNA), which will provide a rich foundation for improving and promoting the health of each community in Cambria and Somerset counties.

The CHNA is a systematic collection, assembly, analysis, and dissemination of information about the comprehensive health of the community. Its role is to identify factors that negatively affect the health of a population and determine the availability of resources within the community to address those factors.

As part of this assessment, a community health survey was distributed in May of 2025. The CHNA was mailed to random households in both counties, sent out electronically, and hard copies were distributed to local health and human services agencies. A community health summit was held in August to discuss additional data, the results of the survey, and develop action plans around opportunities identified through the survey process.

As part of the Mindful Pause program with Outdoor Odyssey, we are bringing a Mindful Moment to you each month. We hope these simple techniques can help you find regulation and calm as you navigate through life’s daily stressors.

Mindful Moment: Breathe and Widen

Let’s take one minute together.

Sit comfortably and gently close your eyes if that feels safe.

Take a slow breath in through your nose…

and a long, steady breath out.

Take another breath in through the nose…inhale calm…exhale tension.

Each slow breath helps regulate your nervous system and expand your capacity to respond not react.

Now silently ask yourself: ‘What do I need in this moment?’

There’s no fixing. No judging. Just noticing!

One more deep breath in…and a slow breath out.

Your window just widened.

When you’re ready, gently open your eyes and continue your day.
