Jade is one of our AmeriCorps volunteers for the fall semester. Jade has worked in the medical field for 10 years. She is currently working as an LPN while continuing her education to obtain her BSN at Frostburg State University. This semester Jade is going to be working with our regional college campuses/programs to identify opportunities to hardwire and expand health and human service education in our at-risk communities. She is very passionate about giving back to the community and helping others.

Jade is married and a mother of two, a 7-year-old boy and 5-year-old girl. In her free time, you can find her supporting her children at their many sporting events and school activities.

Kayla is from Ebensburg PA. She is currently a junior at the Indiana University of Pennsylvania majoring in Public Health with a minor in Safety Sciences. CPH Leaders Discuss HUB Program at national conference

This month CPH leaders, Jeannine McMillan and Kimberly Roberts attended Putting Care at the Center, a national conference for health and social care providers, policymakers, consumer advocates, program leaders, payers, researchers, and more who are working to improve care for people with complex health and social needs. Presented by the Camden Coalition’s National Center for Complex Health and Social Needs, this year’s annual event was held in Pittsburgh and was co-hosted by UPMC Health Plan. The Camden Coalition’s mission is to improve the health and well-being of people with complex needs by demonstrating and advancing equitable ecosystems of care. Their vision is transformed health and social systems that ensure every individual receives person-centered care rooted in authentic healing relationships.

Jeannine and Kimberly participated in the Exploring the Community Care HUB model: Examples from Pennsylvania and New Jersey panel discussion. The discussion was moderated by Pathways Community HUB Institute’s (PCHI) Amy Vreeland, who explored the importance of creating networks of community-based organizations using the PCHI HUB model of care. Jeannine and Kimberly were joined by Kimberly Birdsall, Executive Director of a New Jersey PCHI HUB. During the engaging discussion, Jeannine and Kimberly highlighted the work of the HUB and provided an overview on how the Community Health Worker’s data collection drives the CPH’s systems-level efforts in the community.
