CPH honored with Highmark Bright Blue Futures Award

CPH was selected for the Highmark Bright Blue Futures Award for its impactful work via the Community Care HUB, which utilizes the Pathways Community HUB Model to coordinate care for individuals with complex health needs. The HUB’s focus populations include pregnant people, parents of children aged 0-18 months, two school district populations, and those experiencing substance use disorder.

“It was an invaluable opportunity to connect and exchange ideas and best practices with those who share our passion for improving community health," said CPH Data Analyst Leanne Lovejoy. "This award reinforces our commitment to improving health outcomes and to continue driving progress in our region through the Community Care HUB.”

CPH staff attends CHW Conference

Community Care HUB staff Kimberly Roberts and Amy Bunn, along with HUB Community Health Workers Justina, Tracey, Josie, Stephanie, Christy, and CHW Supervisor Samantha attended Pennsylvania's Third Annual Community Health Worker Conference in State College on September 23 and 24.

CPH partnering with Westmont Hilltop to hold Back-to-School Health Fair

CPH and Westmont Hilltop partnered for the second year to host the Back-to-School Health Fair. Approximately 25 organizations attended to hand out resource information.

AmeriHealth Caritas brought their mobile unit, the Cambria County Drug Coalition had a large inflatable brain that attendees walked through to see how drug use affects it, and the Johnstown Tomahawks and Johnstown Mill Rats even attended.

Local Food Systems Coordinator holds 'So Many Tomatoes' event at library

CPH Local Food Systems Coordinator Nan McNinney worked with Adagio Health to hold a "So Many Tomatoes" cooking event at the Cambria County Library.

Three tomato recipes were provided to attendees.

McNinney held a similar event at The Women's Help Center.

What does being a Community Health Worker mean to you?



Being a Community Health Worker means that I can help people in the community achieve a healthier, stable lifestyle. We are the support system that someone may need. There are a bunch of resources available in our area that most people are not aware of, so being a CHW allows me to show them what’s available.

September is Suicide Prevention Awareness Month

Suicidal thoughts can affect anyone. Age, gender, and background don’t matter when it pertains to suicidal thoughts, much like they don’t matter when it comes to mental health conditions.

September is Suicide Prevention Awareness Month and, this year, September 10 is World Suicide Prevention Day.

Wash and Tune scheduled for October 4

The Wheelchair Wash and Tune event is October 4 from 10 a.m. - 2 p.m. at the Frank J. Pasquerilla Conference Center.

The event's purpose is to bring attention to the health and safety of the wheelchair user and their chair.

Wheelchairs will be cleaned, and minor repairs will be made.

The event is free.

At both events, a variety of resources were handed out to attendees.
