Community Care HUB expands to help Family Medical Center patients

The Community Care HUB, an initiative of Center for Population Health, has expanded its community health worker program to serve patients at the Conemaugh Family Medical Center (FMC).  


This expansion is made possible through funding support from Magellan Healthcare. Eligibility for the HUB program through FMC is a Cambria County resident receiving medical assistance, a mental health or substance use diagnosis, and in need of social services and/or health resources.

Hope 4 Johnstown holding Community Fun Day

Hope 4 Johnstown will be holding a Community Fun Day on Saturday, June 29 from 11 a.m. to 2 p.m. at Eastside Elementary School.


The event is a great opportunity for families to have fun and learn more about resources they may be eligible to receive.

CPH staff provide resource information at Johnstown Farmers Market

CPH Local Food Systems Coordinator Nan McNinney (above) and CPH Communications Coordinator Allie Byers (below) provided resource information at the Johnstown Farmers Market in June.


The Farmers Market takes place every Friday through October 25.


SNAP at the Farmers Market will also take place every Friday. SNAP users are eligible for "Fresh Bucks" to match a portion of their purchase. Stop by the token table for more information.

WalkWorks Wednesdays came back to downtown Johnstown

CPH once again partnered with Cambria Regional Chamber to bring WalkWorks back to downtown Johnstown.


The 1.8-mile route is walked by participants at noon on Wednesdays for a six-week period.


The walks are a great opportunity to meet new people and take in the sites of the city.

CPH sponsoring Health and Social Services Fair at Johnstown Mill Rats game

Center for Population Health is sponsoring a Health and Social Services Fair Saturday, June 29 at the Johnstown Mill Rats game.


Visit the resource tables on the concourse before the game at 7 p.m.

Community Help Center's community garden beds installed

The Community Help Center's raised garden beds and gazebo were installed.


CPH Local Food Systems Coordinator Nan McNinney helped secure funding for this initiative through Community Foundation for the Alleghenies and AmeriHealth Caritas.


A planting day will be planned in the future.

CPH has open AmeriCorp position

Center for Population Health has an open AmeriCorps position.


The community wellness outreach position will focus on connecting community members with essential food and health resources. Interest in public health, education, communications, and/or community outreach required.


The position is part-time (100-hour contract, approximately 9 hours per week).
