South West Self Management Program

The South West Self Management Program is housed within the Thames Valley Family Health Team, funded by Ontario Health and serves the communities of Grey, Bruce, Huron, Perth, Oxford, Norfolk, Elgin and London/Middlesex. The goal of the program is to have a coordinated approach to support people living with chronic conditions, caregivers and health care providers with self management support, information and training.


What is Self-Management?

Self-management is the individual’s ability to be the director of their care. They have the knowledge, understanding and resources to make decisions around their health (physical, social and emotional health) working in partnership with all members of their health care team.


Improving your health, together.

Providing self-management support through skills workshops, education and skills training. There are a variety of free online self-management workshops available this Spring.


Some of the topics include Powerful Tools for Caregivers, Using the BRAIN to Make Decisions, Living a Healthy Life with Chronic Conditions, Living a Healthy Life with Chronic Pain.

Did you know...?

All Cheshire front line employees attend “Client Centred Circle of Care” education sessions at hire and every 3 years thereafter.  During these sessions, staff break into groups and reflect on the Cheshire Mission Statement and the organizational tag line “Making Independence Possible.”


While in their groups, staff are asked to come up with a tag line they feel reflects how they and others in the community may see Cheshire services in 5 words or less.

Did you know …?

Cheshire, along with help from Western Occupational Therapy students, will be launching a series of education sessions for our front-line employees this year. 


As part of our Strategic Plan, the sessions “Back to Basics” will enhance the quality and integrity of our services focusing on the importance of doing the little things well.  

myCheshire Client Portal


All Cheshire consumers who receive personal support service from Cheshire have access to the myCheshire Client Portal which launched in October 2023.


The Consumer Portal is a great tool for consumers and their families to see their scheduled services along with who is coming for their bookings.


Cheshire has created a training guide for clients and their families who want to improve their usability with the myCheshire Client Portal. The training guide that our clients and their families can use which shows step-by-step instructions on how to navigate the site and complete actions such as viewing your schedule. The guide will continue to be updated as new features and changes are introduced.

Alternate Funding & Equipment Sources


Alternative Funding in Ontario – 211.ca

If you require support, we encourage you to reach out to Ontario’s 211 service to locate other possible resources or funding alternatives. Please call 2-1-1 or visit their website. 211 is a free, confidential and nationwide helpline available by phone in 150+ languages designed to have Navigators easily connect you with the social services, programs, and community support you need. In some areas, 211 is also available by text, phone, chat, and online referral.


March of Dimes Home or Vehicle Modifications

List of Eligible Adaptations and Devices

  • Devices to support basic activities of daily living (bathing, toileting, transferring) relating to mobility
  • Floor patient lifts 
  • Environmental controls (i.e., operate appliances; switch lights and computers on or off; facilitate the use of a telephone; or to open, close and lock doors)
  • Repairs to electrical or battery-operated equipment that are not covered by warranty

 

List of Eligible Construction Projects

Please note that equipment is often the most basic and essential solution to removing barriers. Construction projects will only be considered as a last resort and must be recommended by an Occupational Therapist (OT). In order for a construction project to be reviewed for eligibility, an OT must detail basic and essential equipment that has been trialed and explain why it was not suitable. 

All applications for outdoor lifts and ramps (where concrete is required to be poured) will only be accepted between April 1 to October 31. Concrete ramps are not eligible. Construction of a ramp will only be considered when all other attempts to use a modular ramp have been made and documented by an Occupational Therapist. 

 

During construction, if any issues arise such as but not limited to water damage, mold, mildew, termites, items not to provincial code or safety standards, any electrical or plumbing issues those are the responsibility of the home owner, not March of Dimes Canada. 

List of eligible construction projects include:

  • Modular Ramps
  • Vertical Platform Lifts
  • Stairlifts
  • Ceiling Track Lifts
  • Kitchen adaptations (i.e., rearrangement and lowering or raising of kitchen equipment and cupboards)
  • Bathroom adaptations (i.e., zero-threshold shower, door widening, waterproof shower panels, grab bars, handheld shower on a slide bar, anti-slip flooring, wheelchair accessible sink, accessible toilet)
  • Note: When a bathroom modification is required, but the bathroom structure cannot accommodate a zero-threshold shower, the Program would only consider funding transferable equipment (i.e. a portable roll-in shower) as recommended by an Occupational Therapist).

 

List of Eligible Vehicle Modifications, Adaptations and Devices

  • Transferable equipment such as hand controls, foot controls, lifts, safety devices etc. These should be fully convertible, if possible, to any type of vehicle so that they can be transferred when the applicant changes vehicles
  • Specialized seating and related accessories required for transportation purposes
  • Remote control devices to open garage doors for persons who use wheelchairs, scooters, or who walk with difficulty
  • Raising the roof of a vehicle
  • Lowering of the floor of a vehicle
  • Barrier-free modifications to garages or carports
  • Further vehicle modifications required to meet the child's changing needs
  • Repairs to electrical or battery-operated modifications that are not covered by warranty  

 

What we do not fund

  • Reimbursements
  • Concrete ramps 
  • The purchase of a home or vehicle 
  • Construction projects for newly built homes 
  • Therapeutic or supportive care-related items
  • Cosmetic improvements, upgrades or repairs to the home
  • The construction of additional rooms
  • Household appliances 
  • A second accessible exit (not required by the Ontario Building Code)
  • Hospital beds
  • Suction grab bars
  • Walk-in Tubs
  • Restocking Fees and Return Fees
  • Self-Install Stairlifts
  • Vehicle upgrades and repairs

 

Ontario Disability Support Program (ODSP)


Veterans Affairs Canada (VAC)

If you qualify for the Treatment Benefits program, you will receive a VAC healthcare card.


This healthcare card provides coverage for such things as home health or hospital services, nursing services, appointments with specialists (such as physiotherapists, audiologists, and mental health providers), medical equipment, prosthetics, and prescriptions.


The extent of your coverage will depend on a number of factors, including how you qualified, your health needs and your individual circumstances.
