Program Information Condensed Update Form

Child Care Aware of New Hampshire (CCAoNH) powered by Southern New Hampshire Services strives to provide families with high-quality child care referrals in New Hampshire. It is essential that our database be current and accurate in order for us to best serve families and your program.  
As rates change and vacancies need to be filled, we do a Condensed Provider Update of our early childhood programs every Fall. Please take a moment to complete the 2019 Program Information Condensed Update Form. Programs can choose to fill in the form that was mailed to your program, print the printable form below or use the fillable form below. Then mail, email or fax the Program Information Condensed Update Form to Child Care Aware of NH. 
We thank you in advance for completing and returning the form to us.

Child Care Aware of NH created a Starting the Conversation section in the At-A-Glance e-newsletter. Every month, we will bring you a new resource for best practices, strategies or hot topics that can start the conversation in your program and with your colleagues . We hope you enjoy this section.

Helping Babies Sleep Safely
"For Sudden Infant Death Syndrome (SIDS) Awareness Month, CDC is sharing tips parents and caregivers can use to help reduce the risk of sleep-related infant deaths, including SIDS. Start by placing baby on his or her back for all sleep times—for naps and at night. And share your room with your baby, but not your bed. Learn more about these and other steps you can take to create a safe sleep area for baby in a feature from CDC:  Helping Babies Sleep Safely ."
