Dear Families,

 

Please provide a copy of James most recent health assessment and immunizations.

 

 

In order for your child to continue to be enrolled in our program, your child's health assessment  and immunizations must be received by JULY 12,2018

 

 The last health assessment on file is dated 

06/28/2017.

Important Reminder!

 

 

 

1. HEALTH INSURANCE INFORMATION

If  you do not have Health Insurance Coverage for your child, you must let us know.  If you do, we need the insurance carriers name and policy number.

 

2.  HEALTH ASSESSMENT

Every child under the age of two years old should have a Well Visit every 6 months. After age two, children should have a Well Visit annually. DPW regulations requires licensed child care centers to comply with these standards (www.dpw.state.pa.us).

 

After reviewing our files, your child needs an annual Well Visit and updated immunizations.

 

In order for your child to continue to be enrolled in our program, you will need to provide our office with a copy of a more recent Health assessment by JULY 12,2018
