On an as needed basis a "CLINICAL ALERT" is issued to bring to your attention critically important measures or issues. The measures highlighted in this alert are measures that are included in the Delivery System Reform Incentive Payment (DSRIP) Program. You may also be accountable for these measures through other quality programs with payers. 

In the DSRIP program, Measurement Years for performance start on July 1 and end June 30. We are currently in DSRIP Measurement Year 5 (MY5), which encompasses the period of July 1, 2018, through June 30, 2019. The following measures are collected via the Clinician and Group Consumer Assessment of Healthcare Providers and Systems (CGCAHPS) surveys administered by the Department of Health (DOH) to the Medicaid population ages 18 to 64.

Patients with a qualifying outpatient visit between now and the end of the measurement year may be randomly selected by the DOH and asked the below survey questions. We ask your assistance in ensuring that within your practice the following measures are addressed for all eligible patients as applicable for this measurement year.

Medical Assistance with Smoking and Tobacco Use Cessation - Discussed Cessation Medication
At every visit, discuss and offer only FDA approved tobacco cessation medications that can assist with their quit attempt.
