OPWDD COVID Notification Easy Reference Chart
OPWDD has released a chart that will serve as a quick reference to identify the COVID-19 statuses that require phone notification and when only IRMA entry is required. Please ensure it is distributed to all parties assigned to complete COVID-19 notification. 
This guide should be used in association with the guidance document Positive COVID-19 Notification & IRMA Requirements for OPWDD Providers Updated 12/30/2021 which was emailed to your agenciesā€™ email addresses on December 31, 2021.
