Closing the Loop:

A More Connected Path to Services Across Key Campus

For individuals seeking help, a referral should go beyond a simple handoff.

It should provide a clear and supported path to the next service, while offering our teams visibility into whether that connection is successfully completed.

That’s where a closed-loop referral system comes in.

On July 29, Keys to Change and our Key Campus partners celebrated the launch of the first phase of our new closed-loop referral system through Arizona’s social determinants of health referral network powered by Unite Us – called CommunityCares.

Through an easy-to-use dashboard, partners can create and accept referrals, document services and outcomes, and connect clients directly with other Campus partners when additional support is needed. All in real time, all on one seamless platform.

This new system will allow us to identify where referrals stall, uncover barriers, decrease wait times, and minimize the risk of anyone falling through the cracks.

The first round of partners and programs are now live on the platform:

Additional Key Campus partners will be onboarded as part of Phase Two activities.

Can't Stop Won't Stop

For the second time this summer, Key Campus is further extending its hours of operation as an emergency response to the extreme heat.

Effective August 4 through September 30, The Lodestar Day Resource Center (LDRC) is now open until 10:00pm daily for service connections and respite; Key Campus Outdoor Plaza is now open to unsheltered Key Campus clients 24/7; and LDRC Day Room Bathrooms are now open 24/7.

We're so grateful for the Keys to Change staff, partners, and contractors who are stepping up to make this schedule change possible.

One Man's (or Woman's) Trash...

When a collision with an uninsured party rendered her Mustang un-drive-able, April found a way to make a difference with her totaled car.

Rather than scrapping it for parts, April donated it to a cause she believes in.

After working in the field of behavioral health, donating her vehicle to Keys to Change felt like the perfect way to turn trash into treasure.

Through Keys to Change's Car Giving Program, we take care of everything, including a free tow and donation receipt.

Key Campus is now easier to navigate!

Introducing our brand new directory sign - placed at the entrance of the Key Campus to help guide the way for our clients and guests.

The 13-acre Campus is designed to remove barriers to accessing services - and it's more accessible than ever before. From back-end tech solutions to front gate directory signs, we're putting our energy into improving the client experience at Key Campus.

Community Shoutout

Keys to Change recently received a generous investment from the Diane & Bruce Halle Foundation directed to the Street Outreach Lodestar Day Resource Center Supportive Services teams.

With this grant, Keys to Change will focus efforts on reaching the unhoused around Key Campus, offering immediate relief and services, and extending housing navigation.

Together, Keys to Change and the Diane & Bruce Halle Foundation are helping those experiencing homelessness on their journey home.
