How to Complete Requirements for the Pay-Per-Call Program – First Quarter Payout
The first quarter payout reporting period will close on October 5, 2025. To ensure proper processing of payment, follow the steps below:
Step 1: Complete Program Requirements
- All items set forth in the Pay-Per-Call policy must be finished by the deadline, including:
- Call Reports- 100%
- Vector Solutions Assignments- Completed as assigned, through the month of September.
Step 2: Submit Your W-9 Form
- Fill out the attached W-9 form with your most up-to-date mailing address.

Email your completed form to Taylor Lunsford at: tlunsford@botetourtva.gov
- Deadline: Friday, September 26, 2025
**Important:
- If you change your mailing address at any point during the year, you must submit a new W-9 form.
- Incorrect or outdated addresses will result in checks being mailed to incorrect addresses. Any check returned due to an incorrect address, will not be reprocessed.
Declining Participation (Optional)

If you do not wish to receive payment or participate in the program, complete the attached Declination form.

Submit it to tlunsford@botetourtva.gov by Friday, September 26, 2025.
Key Dates to Remember
