Hello! The Center for Independent Living Options (CILO) would like to share an important opportunity for people with disabilities, their loved ones, and community professionals in Cincinnati and Northern Kentucky. CILO is conducting a needs assessment to better understand people with disabilities’ quality of life and access to services in the following areas: health, wellness, housing, transportation, and finances. Additionally, we are interested in learning more about how CILO can customize our programs and services to best meet the needs of our consumers and broader disability community. Those who complete the needs assessment will be entered into a drawing to win a $25 gift card!

Individuals of all ages and disability types are welcome to complete the survey. Disability service providers or other community professionals are welcome to complete this survey as well. This survey is anonymous and confidential. Your name will not be attached to your answers unless you decide to add it to your response, and your answers will not be shared with anyone outside of CILO.
