The Villas have collaborated with SilverSage Physician Services , a premier physician management company and they are delighted to work with Dr. Toby Smith as their on-site physician.
In 2018 Dr. Toby Smith was selected as faculty for AMDA’s Core Curriculum in Medical Directorship. 
SilverSage is excited that Dr. Smith,
based on our most recent research from The Villas, will be presenting a poster at AMDA this March entitled -
Proof of concept: Nursing Home Specialist in Action. The impact of full-time physician services in Post-Acute Care” 
Dr. Toby Smith is the VP and CMO of SilverSage Management Services. Toby L. Smith, DO, CMD earned his doctorate degree in osteopathic medicine from Western University of Health Sciences, College of Osteopathic Medicine of the Pacific. He also holds a Master of Science degree in Kinesiology from California State University Northridge and a Bachelor of Science degree in Sports Medicine from Pepperdine University.
Prior to entering medical school Dr. Smith spent 5 years working in nursing homes as a research associate for UCLA’s Borun Center for Gerontological Research. He worked one on one with nursing home residents and is where he developed a love for geriatrics and taking care of nursing home patients. He pursued this passion by completing residency in Internal Medicine and fellowship in Geriatric Medicine at the University of Hawaii. He was eventually hired by Dr. Scott as one of the first full time nursing home physicians to work for Life Care Physician Services, LLC.
In his role as a full-time nursing home physician Dr. Smith became certified by AMDA: The Society for Post-Acute and Long-Term Care Medicine as a Certified Medical Director and has served in this capacity for multiple facilities for the past 5 years. He was awarded Life Care’s Divisional Physician of the Year in 2012. 
Dr. Smith joined Dr. Scott in his dream to improve the quality of care in nursing homes in 2017.
Dr. Smith is board certified in Internal Medicine, Geriatric Medicine and Hospice & Palliative Medicine. He is an assistant professor at the University of Hawaii’s Department of Geriatric Medicine and serves as the Vice President of the Hawaii chapter of AMDA: The Society for Post-Acute and Long-Term Care Medicine. He has been an author of multiple publications and has presented at national conferences throughout the country. He is a current member of the American Medical Association, the American Geriatrics Society and the Christian Medical and Dental Association.
Jenni is a 3 rd grade teacher at Makua Lani Christian Academy
The Smith family love the Island life.
They enjoy the outdoors, beach and ocean, and all the family members are avid swimmers and surfers.

HOKULEA: HISTORY OF HAWAII’S VOYAGING CANOE
New Voyages
Pride, heritage, and adventure. These may have been but a few of the reasons the Hokule’a was constructed in the early 1970s. The Hokule’a, meaning our Star of Gladness, is a 62-foot double-hulled sailing canoe built by the Polynesian Voyaging Society. It was constructed as a restoration project, only instead of restoring an object, it was helping to restore a culture many Hawaiians felt was being lost. “The voyaging canoe was built in order to challenge the notion that Polynesians had discovered and settled the Pacific islands by accident.” 1 Using ancient sailing techniques, the Hokule’a has enjoyed numerous successful voyages, ushering in a new era of Hawaiian cultural pride. The Hokule’a is a symbol of what can be accomplished by trusting in ourselves, honing techniques that work, and by forging bravely ahead through uncharted and unknown waters.
                We at SilverSage are sailing unknown waters of our own, filled with challenges and obstacles. We are challenging decades long practices and traveling new, uncharted waters. As we hone our techniques of quality care and strive for new friendships, we look to the story of the Hokule’a and the pioneers of the past it represents. If the ancient peoples of Polynesia can find islands in the middle of the Pacific using only the tide and the stars, we can create a level of quality the long-term care community has never seen before. - -Evan Johnson-

Last year the Smith’s surprised the Scott’s with an Out-rigger canoe adventure to experience a manta ray swim like no other, with a small group of six on a double hull canoe .We enjoyed an authentic Hawaiian experience in our paddle-powered canoe with one of the ocean’s most graceful creatures… t o our surprise the captain of our boat was one of the team members on an all-women’s team that took part in the sailing adventure of Hokulea! …
