Dr. Toby Smith

Toby L. Smith, DO, CMD
earned his doctorate degree in osteopathic medicine from Western University of Health Sciences, College of Osteopathic Medicine of the Pacific. He also holds a Master of Science degree in Kinesiology from California State University Northridge and a Bachelor of Science degree in Sports Medicine from Pepperdine University.
Prior to entering medical school Dr. Smith spent 5 years working in nursing homes as a research associate for UCLA’s Borun Center for Gerontological Research. He worked one on one with nursing home residents and is where he developed a love for geriatrics and taking care of nursing home patients. He pursued this passion by completing residency in Internal Medicine and fellowship in Geriatric Medicine at the University of Hawaii. He was eventually hired by Dr. Scott as one of the first full time nursing home physicians to work for Life Care Physician Services, LLC.

Dr. Smith
became certified by AMDA: The Society for Post-Acute and Long-Term Care Medicine as a Certified Medical Director and has served in this capacity for multiple facilities for the past 5 years. He was awarded Life Care’s Divisional Physician of the Year in 2012.

Dr. Smith
is board certified in Internal Medicine, Geriatric Medicine and Hospice & Palliative Medicine. He is an assistant professor at the University of Hawaii’s Department of Geriatric Medicine and serves as the Vice President of the Hawaii chapter of AMDA: The Society for Post-Acute and Long-Term Care Medicine. He has been an author of multiple publications and has presented at national conferences throughout the country. He is a current member of the American Medical Association, the American Geriatrics Society and the Christian Medical and Dental Association.
