Asian Americans, Native Hawaiians, and Pacific Islanders (AANHPIs) Heritage Month


In May, we reflect and celebrate the important role that Asian Americans, Native Hawaiians, and Pacific Islanders (AANHPIs) have played in our shared history.


The Federal Asian Pacific American Council (FAPAC) is pleased to announce the theme for the May 2025 Asian American, Native Hawaiian, and Pacific Islander (AANHPI) Heritage Month, “A Legacy of Leadership and Resilience.” 


Here are some ways you can celebrate and observe:

Jewish American Heritage Month


In May, we celebrate more than 350-year history of Jewish contributions to American culture!


According to Jewishamericanheritage.org

celebrating Jewish American Heritage Month helps to strengthen our communities and promote a sense of belonging, shared civic life, and a deeper understanding of cultural heritage. Explore unique resources from across the United States.

Military Appreciation Month



May is Military Appreciation Month, a special time for those in and out of the military.


This month we honor and recognize the contributions, sacrifices, and service of the members of the armed forces, past and present.


Not only do we pause on Memorial Day to remember the sacrifice and service of those who gave all, but the month also marks several other military anniversaries and events, including Military Spouse Appreciation Day and Armed Forces Day.


Here are some ways you can observe and honor those who have served and their families:


CalFresh Awareness Month



May is CalFresh Awareness Month in California and the San Diego County Health and Human Services Agency along with its community partners will be at several community events where people can receive free screenings and application assistance for CalFresh.


CalFresh is California’s version of the federal food assistance program, SNAP, and has been around since 1964. The program is designed to help families and individuals increase access to fresh and healthy food, part of the County’s Live Well San Diego vision for healthy, safe, and thriving communities.

COMMUNITY SPOTLIGHT

Women, Infants, and Children (WIC) New Referral Pathway Tool


What is WIC?

WIC helps families across the state by providing nutrition education, breastfeeding support, healthy foods and referrals to health care and other community services.


WIC serves infants and children up to age 5, as well as pregnant individuals, those who have recently given birth, and individuals who have experienced pregnancy loss. Partners, grandparents, foster parents of young children, and working families are also welcome at WIC.


WIC is safe and available for all eligible families, including immigrant families. It is open to anyone living in California who meets the program guidelines and income requirements, regardless of race, color, national origin, gender, religion, age, sexual orientation, gender identity, or disability. Participation in WIC does not impact public charge considerations. Currently, families can access WIC without fear of immigration consequences. 


How to Apply for WIC

Getting a WIC application started is easy:

  1. Go to //sdwic.com/apply
  2. Enter your address to find an office near you.
  3. Select an office.
  4. Fill out the form.


Someone will reach out within 3 business days to continue the application process.

Housing Instability Prevention Program (HIPP)


Offers assistance to help pay rent and other housing-related expenses for households experiencing a housing crisis and at risk of homelessness. Currently only providing priority to populations, such as:​​

• Participants with disabilities​​

• Families with children (ages 0-17)​​

• Transitional-aged youth (TAY), ages 18-24​​

• Heads of household aged 55 and older​​

​​

Also offers a flat-rate shallow subsidy for up to 24 months. Provides assistance for housing-related expenses such as past-due rent and past-due utilities. The goal of the program is to achieve housing stability independent of the subsidy.

Youth Systems Navigation Program


This program provides one-on-one housing navigation, benefit enrollment assistance, employment support, educational guidance, and emergency food resources for transitional age youth (TAY). Helps youth access safe and stable housing, navigate the coordinated entry system, and connect to supportive services tailored to their individual goals. The program uses a wraparound approach to promote long-term stability, independence, and well-being.

Civitas’s Annual Conference brings together hundreds of passionate industry leaders, decision-makers, and innovators to discuss interoperability, data-driven quality improvement, social determinants of health, public health, multistakeholder and community-led governance, and health outcomes. Civitas’ members and network serve as the bridge between “data” and “doing” in health and health care. Attendees choose the conference to foster deep connections, strengthen and build new partnerships, and leave with tangible ways to evolve their national, state, regional, and local initiatives.


Representatives will join us from regional and state HIEs, RHICs, CIEs, QIOs, health data organizations, CBOs, federal, state, and local governments, and business and technology partners.

The CIE Partner Network works to improve the overall service delivery system for clients. Our meeting provides a forum for partners to share updates about ongoing CIE Initiatives, share trends in our community, and collaborate on how we as a network are leveraging our partnership, data sharing, and the CIE technology platform to address the social determinants of health and increase health equity.


Join us for a range of content. Each month, we may dive into the latest technology projects, elicit feedback on the platform’s care coordination tools, or spotlight programs and supports for unique populations. The partner gathering is a space to engage with peers across our shared network. Join us on the 4th Thursday of every month (except November and December) at 9:00 am via Zoom.
