Tip #20: Conduct Your Own CAHPS ® Survey
If you want to learn more about your patients’ experiences, you can field a CAHPS Clinician and Group survey customized to your specific needs and circumstances. This is a great way to get granular data about your practice or individual providers. 

The Agency for Healthcare Research and Quality (AHRQ) offers the survey in Word and PDF versions, along with sets of supplemental items for customization.

AHRQ also has several guides and tips to help you modify the survey to meet your practices’ needs, administer it to patients and analyze the data.  

Visit AHRQ's CAHPS Clinician and Group Survey section to access all of these materials. If you do not have the resources in your practice to manage the survey, there are also vendors available to field surveys on your behalf.
