Connect IRL with GaHIN

We are going back to the future at GaHIMSS Annual Conference on October 7 where Dr. Denise Hines will be speaking on the panel, Driving Digital Revolution Breakout: Biff Took My Job! The Rise of the Resistance in the Future of Work. For more information, click here: Agenda - GAHMISS 2025 Annual Conference.

We will be presenting at the GaDBHDD Behavorial Health Symposium on October 2nd. Our panel will be on Linking What Matters: How Georgia is Advancing Whole-Person Care Through Data Unity.

Georgia HMIS Now Connected to GeorgiaUnify

GaHIN has successfully connected the Georgia Homeless Management Information System (HMIS) with GeorgiaUnify — closing critical gaps in patient information and linking healthcare providers, payers, and wellness partners.

Partners for Home, the Atlanta CoC, was instrumental in making this first connection a reality. "This milestone marks the culmination of extensive collaboration and planning, and we’re excited to move forward with expanding its impact across the state," said Jeanette Pollock, HMIS Projects Manager, Georgia Department of Community Affairs.

Next up: working with CoCs statewide to bring social services and clinical care data together for stronger coordination, better outcomes, and lower costs.

Better Outcomes Start with Connecting to GaHIN

Better care starts with a complete picture. GaHIN connects healthcare providers, social services, and community partners so patients receive coordinated, personalized care without gaps or delays.

1. See Beyond the Chart--Link medical records with social data to address the factors that truly impact health.

2. Get the Whole Story--Access a patient’s statewide record for faster, smarter decisions.

3. Share Without Barriers--Exchange information securely across hospitals, clinics, public health, and social services.

4. Keep Care Moving--Ensure vital details follow the patient through every transition.

5. Strengthen Your Network--Work hand in hand with community partners to tackle root causes of poor health.

When you connect with GaHIN, you are not just sharing data. You are building Whole Person Care.
