2-1-1/CIE San Diego is providing CalAIM Navigation services for CIE partners! CIE partners who have clients who are potentially eligible for Enhanced Care Management or Housing Transition Navigation Services (a CalAIM Community Support) can be referred to 211/CIE for support in screening the client and connecting them to a CalAIM provider.
Enhanced Care Management (ECM) is a Medi-Cal benefit that provides comprehensive care coordination for high-need individuals with complex medical and social needs. ECM connects members with a dedicated care manager who works directly with them to coordinate healthcare services, connect them to community resources, and help address social factors affecting their health. ECM also ensures connection and usage of CalAIM Community Supports services when necessary.
Download the CalAIM desk aid to learn more about who is eligible and how you can refer your clients to this service.

Civitas Annual Conference
Bridging Data and Doing
In Partnership with 211 San Diego/CIE and
Civitas’ California members
| | |
Civitas’s Annual Conference brings together hundreds of passionate industry leaders, decision-makers, and innovators to discuss interoperability, data-driven quality improvement, social determinants of health, public health, multistakeholder and community-led governance, and health outcomes. Civitas’ members and network serve as the bridge between “data” and “doing” in health and health care. Attendees choose the conference to foster deep connections, strengthen and build new partnerships, and leave with tangible ways to evolve their national, state, regional, and local initiatives.
Representatives will join us from regional and state HIEs, RHICs, CIEs, QIOs, health data organizations, CBOs, federal, state, and local governments, and business and technology partners.

March 18, 2025: We attended the 2nd Annual Emilio Nares Foundation: Community Celebration at Green Acre. We had the opportunity to network with other partners, say hello to old & new friends, enjoy yummy bites & beverages, and learned more about ENF’s impact in the community. Thank you ENF for the invitation to attend and celebrate with you!
March 26, 2025: We were invited to attend and host dialogue with students at the annual City Women Rock Conference held at San Diego City College. Student circles included topics like; imposter syndrome, entrepreneurial pathways, community leadership, self-care practices, and so much more. Thank you to our partner for the invitation to be a part of your wonderful programming and for facilitating a connection with students!
April 5, 2025: We had the great pleasure of partnering with The Urban Collaborative Project CDC at the 4th Annual Transportation Justice Expo, held at Gompers Preparatory Academy in Southeast, San Diego. UCPC hosts this event in partnership with so many wonderful community partners and it included; a sustainable fashion show (Instagram: thehomegrownprojectsd), electric vehicle test drives, a micro transit panel, community bike rides, raffle bike & scooter giveaways (grand prize: e-bike) and so many more opportunities for attendees to enjoy.
211SD/CIE hosted an information booth to share more on how clients & community members can access resources and services through our Resource Database and other pathways of connectivity. Our amazing Enrollment Specialist, Marlene Bragg, supported with directly signing up visitors, for phone appointments, via Enrollment Services. In continued partnership with UCPC, CIE facilitated a survey opportunity (as co-designed with our partners in the CIE: Transportation Affinity Group) and compensated participants for sharing their lived experiences, accessing transportation related supports and resources. It was an honor to be in community with so many partners, who are bridging the gap of access and broadening the road to connection. Thank you to The Urban Collaborative Project CDC for having us at this event and for also being a co-collaborator with us!

April 14, 2025: CIE Partner Engagement Director, Zenia Torres, attended the monthly Mid-City CAN: ITCH (Transportation) Committee Meeting. Mid-City CAN hosts these meetings by bring together community members/residents of City Heights, over a meal, to share out important transportation related updates and initiatives. The organization provides training opportunities so community members feel empowered to join advocacy efforts and be champions for what is important to them (for example: extending the MTS: Youth Opportunity PASS Program).
We also have the great pleasure of collaborating with Mid-City CAN via the CIE: Transportation Affinity Group this year and facilitated a compensated survey opportunity at the ITCH (Transportation) Committee Meeting. We are grateful for the committees participation in this lived experience share out and for their trust in this collaborative opportunity!
