The "Lifeplan" which a person with I/DD creates with the help of their Care Manager is critically important when beginning your housing journey. It is the tool by which service needs are determined and goals are prescribed. This Connect session will help you to understand how to use the Lifeplan process to access the services and support you will need to create your own housing plan.
This presentation will be presented by John Maltby, Co-Director of the NY Housing Resource Center.
