The ConnectMyHealth Program Office is pleased to advise that Parents and Legal Guardians of 16-17 year olds, and Legal Representatives of individuals aged 18+, can register to become a Proxy User of ConnectMyHealth. This means the Proxy User may access ConnectMyHealth in place of the Patient to view the Patient's Data; the authorized Proxy is able to use ConnectMyHealth in the same way that the Patient would have otherwise been able to.
Currently, Proxy Access is only permitted when the Patient is not capable of accessing, controlling and managing their own ConnectMyHealth account. For Patients who are capable of using their own ConnectMyHealth account, don't forget about ConnectMyHealth's Record Sharing functionality where a person can share their records with you and vice versa.
