ConnectMyHealth Patient Portal: Features & Functionality Update

NEW! Record Sharing Now Available!

The ConnectMyHealth Program Office is pleased to announce that you can now share your health records with other ConnectMyHealth users, such as your family or caregivers.

Here’s how Record Sharing works!

  • ConnectMyHealth allows you to share your health records with up to 10 ConnectMyHealth users; for example, your family member or caregiver. You can also use your ConnectMyHealth account to view records shared with you by another user.
  • When sharing your records using ConnectMyHealth, you must know the email address associated with the person’s ConnectMyHealth account; if they don’t already have their own account, ConnectMyHealth includes functionality to ‘invite’ the person to register for their own account.
  • You can control what types of health records you share with each person (called a “Shared Access User”). By default, any health records you marked “Confidential” by applying a Consent Directive at a participating hospital(s) will not be shared, however you can decide to share these records as well if you’d like. You also control how long you’d like the Shared Access User (your family, caregiver etc.) to have access to your records for. You’ll make these selections as you complete the Record Sharing process from within ConnectMyHealth.
  • Complete instructions of how to share your records are available from within the portal to guide you through the set-up process.

Loving Having Online Access to Your Health Records?

The ConnectMyHealth Program Office is always looking for success stories and ways that ConnectMyHealth has helped patients. We are very mindful not to collect any Personal Health Information; but rather create quotes and testimonials that can help others learn how they too could benefit by having access to ConnectMyHealth like you have!

Want to share your story? Simply fill out the ConnectMyHealth Testimonial & Consent Form below! As a reminder, please do not include personal health information in your responses. If a written testimonial or quote is prepared based on the information you provide in your form, the ConnectMyHealth Program Office will get back in touch with you prior to publishing the content, and no information that identifies you beyond your first name will be included.

Thanks for your support, and so glad ConnectMyHealth is helping to empower your health journey!

On the other hand, if you find your account is disabled, that may be because you haven’t logged into your account for more than 366 calendar days. Contact the ConnectMyHealth Program Office for assistance in this case; contact information is below.
