Summer break is here, but innovation is in season for Connie! I’m excited to share two service highlights -- the much-anticipated rollout of our patient portal, Connie Patient Connect, and updates on the Connecticut HCBS Program Tool 


As of March, we’ve integrated the CT Home and Community-Based Services (HCBS) Program Tool into the Connie Portal. In partnership with the Department of Social Services (DSS) to improve Medicaid member health outcomes, this resource supports a value-based, fee-for-service delivery model for Medicaid patients through incentive payments to HCBS providers, informed by defined outcomes. In short, to receive incentive payments for providing value-based care, participating HCBS providers measure the progress of patient health goals in order to meet their success metrics, which is where Connie comes in! 


Connie supplies a platform using limited data sets in the Connie Portal to easily track how these organizations are performing in relation to the DSS HCBS program benchmarks, and the HCBS Tool is taking off!


We’ve also officially launched Connie Patient Connect, a first-of-its-kind portal that grants patients access to their own health information across EMRs, at no cost. This is an incredible step forward in empowering patients in Connecticut by making their health information more accessible, transparent, and streamlined. Patient Connect is especially useful for patients with chronic conditions, as they’re often treated by multiple providers who may not share the same EMR, which can delay essential information and care updates.  


Patient Connect brings us closer to achieving our Patient Access goals according to CT State Statute 17b-59d: provide patients with secure electronic access to their health information; and allow voluntary participation by patients to access their health information at no cost. It’s a win-win for Connecticut!

CONNIE PATIENT CONNECT

Connie Patient Connect is LIVE! This secure, easy-to-use, free portal provides patients with access to their health information across multiple providers, regardless of what EMR is used. Using the portal, patients are empowered to be active collaborators in their healthcare journey with their care team, making the portal a meaningful step towards improved health outcomes and a more connected Connecticut!  

 

Our website has a host of resources for you and your patients, including a video walkthrough of how to set up an account, user guide, informational flyer, poster, terms of use document, and privacy policy.

VITALS SECTION AND MEDICAID REDETERMINATION

Patient 'Vital Signs' are now available in the Connie Portal under 'Patient Records' for viewing. To access this new tab, select 'Clinical Data' within the 'Clinical Information' section.  

 

Additionally, Connie’s new Medicaid Redetermination service is now available! By providing a monthly list of Medicaid members who are up for eligibility review, connected organizations can stay ahead of Medicaid coverage lapses and support continuity of care. If this service would be useful for your practice, please reach out to your designated Connie Account Manager!
